I am Joemari P. Fulgencio, a BS Physical Therapy student currently undertaking a study entitled “Effect of
Proprioceptive Exercise on Kick Accuracy AmongSepakTakraw Players with CAI.” This study will help
physical therapists develop a more comprehensive fall-risk evaluation approach and contribute to the growth of
evidence-based physical therapy.

In this regard, may I ask your permission to allow me to pilot test my tools among the patients with chronic stroke
condition? Those who will qualify based on the inclusion and exclusion criteria and consent to participate will listen
to a set ofmusic in any musical genre of their choice that will last for an hour and will be assessed by two (2)
licensed Physical Therapists. Their level of cognitive function will be assessed at the beginning and after every end
of the week of intervention. This process will only take 10 minutes to complete. All forms will be provided for free.
