APPLICATION FOR SEPARATION

(Pursuant to Sec. 10, Art. XVI of RA No. 11054)

I HEREBY APPLY FOR SEPARATION FROM THE OFFICE OF THE REGIONAL

HUMAN RIGHTS COMMISSION -ARMM WHERE I CURRENTLY OCCUPY THE
POSITION OF PLANNING OFFICER II WITH GROSS MONTHLY SALARY OF THIRTY
THOUSAND FIVE HUNDRED THIRTY ONE PESOS (PHP 30,531.00).

I understand that this application shall facilitate my complete separation from the
government service, and agree to the condition set forth in Sec. 10 Art. XVI of RA No. 11054
that “(a)affected personnel who are retired or are separated from the service shall not be re-
employed in any agency of the Bangsamoro Government or the National Government, Including
government-owned or controlled corporations for a period of five (5) years. The retired or
separated personnel who are re-employed during the prohibited period shall refund, on a prorated
basis, the separation incentives they received xxx”.
