AFFIDAVIT OF UNDERTAKING

That I, JUAN DELA CRUZ, 52 years old, Filipino,

married, with residence and postal address at Purok 1,
Barangay 2, Mercedes, Camarines Norte, after having been
sworn to in accordance with law, hereby depose and say;

1) That I am the owner of a motorized banca, F/B Juan

with Mayor’s Permit issued by the Municipality of
Mercedes under permit no. 2019-71;

2) That said fishing boat was apprehended for being

involved in an illegal fishing last 02 August 2019 around
11:00AM at Siruma, Camarines Sur while the said
fishing boat is being piloted by Redondo Braga;

3) That the said fishing boat was impounded by the Siruma

Police for further investigation and custody;

4) That I am respectfully requesting for the release of the

said fishing boat because it is being used as a means of
livelihood for our daily sustenance;

5) That I promise that I shall produce the said fishing boat

before the Siruma Municipal Police Station, or any office
or court that may eventually investigate of hear the case
in connection with the above-mentioned accident;

6) That I also promise that I shall not dispose, transfer or

sell said fishing boat while it is still involved in the
investigation or any pending case in connection with the
above-mentioned accident;

7) That I am executing this affidavit in order to confirm and

attest to the truthfulness of the statements as above-
mentioned and for the purpose of requesting the release
of my fishing boat F/B Juan, and for any purpose that
this affidavit may serve.
 IN WITNESS WHEREOF, I have hereunto affixed my
