MINUTES OF THE MEETING/INVESTIGATION

July 23, 2018, Monday (11:45 A.M.)

Issue:

 A memo was issued to the concerned individuals with regards to the alleged falsification of the
Certificate of Demonstration Teaching that was held in Davao last April 22 – 24, 2015 which
was submitted as one of the requirements for promotion/reclassification.

Present Individuals:

 Mrs. Marineth S. Jalon (MT – I)

 Ms. Ma. Grace R. Collado (MT – II)
 Dr. Helen D. Abad (Principal)

Salient points:

 Dr. Abad read the memo to Ms. Marineth and Ms. Grace as to begin the meeting proper.
 Dr. Abad discussed that she had already consulted Dr. Edsel R. Francisco (Principal IV of
ZCSSC) and Ms. Daisy G. Ebrada (Principal III of SBCS) as to the discovery of the issue with
regards to the submission of the Certificate of Demonstration Teaching.
 Dr. Abad inquired as to the authenticity of the Certificate of Demonstration Teaching.
 In response, Ms. Grace admitted that they have attended a seminar, however such seminar did
not cover any Demonstration Teaching activity, as such no certificate should have been issued.
 Upon discovery of such facts, Dr. Helen informed them of possible consequences if the higher
committee on approval shall investigate the matter at hand.

Resolution:

 Dr. Abad decided to give Ms. Marineth and Ms. Grace a “null and void” for their district
ranking position.
 As for Ms. Marineth and Ms. Grace, they opted to retrieve their promotion paper documents
so as not to alert higher authorities for a deeper investigation and to avoid heavier sanctions.
