Greetings in the name of St. Vincent de Paul!

We, the 4th year students from the College of Pharmacy are currently taking the Thesis Writing
Course and a proposed working title "Attitude And Perception Of Guardians On Children
Immunization In The Adopted Communities Of Adamson University; Basis For Developing And
Implementation Of Healthcare Programs", this is in response to the growing number of children
acquiring disease from preventable causes.

In this regard, we would like to seek for coordination from your good office to consider our
request and intention to pursue our research in these adopted communities of our University,
particular the adopted communities in Bulacan, Laguna, and Rizal.

In line with this, may we kindly request for the updated number's of families in This data shall be
used as the basis of our conducting survey in the future as we further progress our research study.

These perspective locations as a band for determining the sample population to be surveyed. The
result of this study will be able to help the college and University in developing programs that
can contribute to the health and wellness of the community members.
