EXTRACT RECORD OF EVENTS

Hereunder is the Extract Record of Event from Official Blotter Book of Desk
Officer dated September 5, 2019 with entry number 2019-05-09-7140 of page 183 and
recorded by PCpl Gabriel Pague to wit;

2019-05-09-7140 2:05 pm- Aresto Agbong of legal age, married and holding
office at Mati Polytechnic College, Inc. appeared in this station and requested to put into
record that sometime in 2017 Aresio Agbong brought his rifle at the gun service shop of
certain Mr. Vale, unfortunately Mr. Vale was killed and the rifle cannot be found inside
his shop. Mr. Aresio Agbong exerted effort to locate the rifle but such search remain
fruitless.

This Extract Record of Event is being issued upon the request of Mr Aresio
Agbong for whatever legal purpose it may serve him best.

Given this 5th day of September 2019 at Mati City Police Station, Mati City,
Davao Oriental, Philippines.
