She was admitted to the provincial hospital on August 20th for uncontrollable vomiting and stomach complaints and diagnosed with a urinary tract infection. From August 22nd to 24th, she was admitted to another hospital where she was diagnosed with acute gastritis. Her mother Julie Tanya P. Lanzar is requesting the absences be excused and for Quin to be excused from any missed school activities, as evidenced by the attached medical certificate.

    Greetings!

    Please excuse my daughter QUIN AVERY P. LANZAR, a KINDER II PUPIL of your school for having been
    absent for the following days:

     August 20, 2018 (Monday)


     August 22 to 24, 2018 (Wednesday to Friday)

    The reason for her absences were due to her being admitted on August 20, 2018 at the Provincial
    Hospital in Baybay City, and on August 22 to 24, 2018 at Ormoc Doctor’s Hospital in Ormoc City. She had
    uncontrollable vomiting and stomach complaints. The findings of the doctors were at first Urinary Tract
    Infection but after having been transferred to another hospital, she was diagnosed with ACUTE
    GASTRITIS. Attached herewith is her medical certificate of her hospitalization as evidence to further
    support her valid absences.

    I am respectfully requesting that you excuse her from whatever school activities that she might have
    missed.
