JOINT AFFIDAVIT OF COHABITATION

We, LEOH M. SALAS and CATHERYN M. DAQUIPIL, both of legal age,

Filipinos and residents of Purok 2, Libertad, Butuan City, after having been
duly sworn to an oath in accordance with law, hereby depose and say, that:

We have been living together as husband and wife since February of

2013 or for six (6) years under the same house located at Purok 2, Libertad,
Butuan City;

There is no legal impediment for us to get married;

We desire to validate our cohabitation by contracting a marriage;

We are executing this affidavit in lieu of a marriage license under article

34 of the family code;

We are executing this joint affidavit of cohabitation to attest to the

veracity and truthfulness of the foregoing statements and for whatever legal
purpose this may serve.
