AFFIDAVIT OF MINORITY

I, EZMYR NOEL I. BATAIN, married, of legal age, Filipino,

presently residing at Barangay Luta Sur, Malvar, Batangas after having been
duly sworn to in accordance with law do hereby depose and state THAT:

1. I am the biological, true and legal father of AUBREY D.

BATAIN, a minor, whom I am processing the Certificate of Live Birth
pursuant to Republic Act No. 10172, for correction of her sex and/or gender,
attached here as Annex “A” is a copy of her Certificate of Live Birth issued
by the National Statistics Office (NSO) to prove her minority;

2. By virtue of such, my minor child, Aubrey, was required to

obtain a National Bureau of Investigation (NBI) Clearance and Police
Clearance as part of the requirements for the correction of her sex and/or
gender;

3. However, considering her minority, she cannot yet acquire the

said Police and NBI Clearances;

4. I am executing this affidavit to attest to the truth of the

foregoing facts that my child AUBREY D. BATAIN is a MINOR, and for
all legal intents and purposes it may serve.

Affiants further sayeth naught.
