AFFIDAVIT OF DENIAL

I, JOCELYN HERRERA y CASTRO, of legal age, Filipino and

with residence and postal address at Brgy. Natipuan, Nasugbu, Batangas
after having been duly sworn to in accordance with law, hereby depose
and state:

1. That when I filed an application for clearance at the National

Bureau of Investigation, Balayan Satellite Office, Balayan,
Batangas, I was advised that the said office could not issue the
required clearance in view of the appearance of a certain
JOCELYN CASTRO, who has a case for Estafa filed with the
Municipal Trial Court of Pampanga, Br.006 with Criminal
Case No. 8770. In addition, the name JOCELYN CASTRO
also has a pending case for Qualified Theft filed with the
Regional Trial Court of Parañaque Br.274 and a case for Estafa
filed with the Office of the City Prosecutor of San Fernando,
Pampanga with Case No.: OCPSF-07-D-676-682;

2. That I am not the same person who carries the name JOCELYN
HERRERA y CASTRO;

3. That I have never been accused of any criminal activity within

the Philippines;

4. In view of the foregoing, I am requesting NBI Office to issue

the appropriate NBI clearance in my favor for compliance with
the certain requirements.
