To whom it may concern:

This is to justify that the Biometric Machine of Mabini National High School was
corrupted for the following dates; January 3-15, 2019. Furthermore, there was an
electric disturbance on the afternoon of January 28, 2019.

In connection to this, there was no print-out data for the said days. Thus, the Daily Time
Record of the teachers was manually written for the above-mentioned dates. Attached
to their Form 48 are the Photocopies of the Logbook as evidence of their attendance.
