To Whom It May Concern,

Greetings!

The Department of Health – Philippine Centers for Specialized Healthcare (DOH-PCSHC) is one of the
biggest medical internship programs in the country. This is the only internship program which rotates in
five of the leading public and specialized hospitals namely: East Avenue Medical Center, Philippine Heart
Center, Lung Center of the Philippines, National Kidney and Transplant Institute and Philippine Children’s
Medical Center. The rotation in these different hospitals allows medical interns to develop their knowledge
and skills in medicine. This internship program encompasses medical graduates from all over the country.

In line with the Department of Health’s vision of “All for health towards health for all”, the DOH-PCSHC
organizes several medical missions in different far-flung areas in the country every year, which aim to
provide comprehensive medical services to the underprivileged residents of the selected localities. This
year, Team C, which is composed of 16 medical interns, has chosen to reach out to the indigent patients
of Bislig, Surigao Del Sur on October 14-20, 2019. The medical mission will target 1,000 beneficiaries
(pediatric and adult). We will also conduct health education on topics such as proper hygiene, top causes
of morbidities such as hypertension and diabetes mellitus as well as smoking and alcohol cessation, and
other health topics in line with the DOH calendar of events. We have already coordinated with the town
Health Committee chairperson and the local Rural Health Officer of the said locality.

In this regard, we would like to humbly ask the assistance of your good office for the execution and
success of the said medical mission thru the provision of medicines, hygiene kits, food products or
financial aid (for transportation, lodging, and other expenses). As a sponsor, we would gladly
acknowledge your assistance through publication materials (printed and online), AVP of the medical
mission and post-mission report.

We believe that establishing and maintaining a collaborative partnership with your office would be of great
help to us in making this event a success, and more importantly, in making a change towards the
betterment of healthcare in the country.

Should you have any inquiries, feel free to contact Gerben F. Villanueva at 09998836150 or email at

We hope for your favorable response. Thank you and God bless!
