Reasonable Request for Accommodation for Debra Smith:

Decision Committee:

I am submitting this request of behalf of Debra Smith, who has been attempting to lease up in
Snohomish County using a NED voucher. Debra has a health condition that required a surgery in
the past few months, which limited her ability to actively participate in a housing search.

Debra is working with a very experienced Community Choice Guide and also has the support of
his DSHS CM in continuing the search. I am asking for a 90 day extension to give Debra more
time to find a unit that will accept the NED voucher, now that she is in recovery.
