do name, constitute and appoint, JUAN DELA CRUZ of legal age, married and a resident of
Tuguegarao to be our true and lawful attorney-in- fact, with power and authority to do and perform
the following acts:

1. To claim and receive the atm card issued by union bank

2. To execute and sign legal documents relative to the atm card;
3. To deliver, possess, execute and sign legal documents to accomplish the foregoing
purposes.

authority to do and perform all and every act and thing whatsoever requisite or necessary and
