AFFIDAVIT OF LOSS

I, LEX REDEN EMMANUEL O. ENAMAN, of legal age, married,

Filipino, and a resident of Victoria Subdivision, Barangay Caggay,
Tuguegarao City, Cagayan, and after having sworn to in accordance
with law, hereby depose and state:

1. That on September 3, 2016, in the morning, I was then in front of

CSU Library, CSU Campus, Carig, Tuguegarao City, Cagayan,
waiting for our professor to arrive;

2. That moment later, I went outside the School Campus to eat my

breakfast, it was then that I notice that my school ID [
Identification Card ] which I usually hang on my neck as a neck
lace was no longer in its lace;

3. That I exerted efforts to locate that same but proved futile;

4. That I believed that the same is now beyond recovery;

5. That I am executing this affidavit in order to attest to the truth

and veracity of the foregoing statements and for the school to re-
issue a new one for and in my name.

FURTHER AFFIANT SAYETH NAUGHT.
