SAFETY POLICY: We can always restart work - we cannot eliminate an accident after it has happened.
SAFETY GOAL: ZERO ACCIDENTS

SHORT CUT LEADS TO


DEAD END

Some portion of the project have now been handed over to United after successful
completion of pre-commissioning test procedures ranging from hydro-testing, pneumatic
testing, chemical cleaning, etc.

During this period when other areas are being prepared for turn over, taking the easy way
out by means of non-compliance to work permit requirements or non-compilation of the
Safe Work Plan are examples of short-cuts that ultimately result to no way out but DEAD
END.

As many areas on site are getting ready for various pre-commissioning activities, through
the initiative of their management, and as more completed areas are being handed over to
UNITED the need to effectively prepare the mind-set of the people was recognized and
addressed before actual work finally commences.

The elements of this feature article may be taken as path-forward initiative to optimize
safety and productivity and complete the work without any untoward incident.

HOUSEKEEPING
 Daily removal of small pieces of waste materials including excess materials on scaffoldings and on
permanent steel gratings.
 Supervision to ensure that tools are not kept or left inside open end pipes, before quitting time.
 Redundant and waste materials are removed from the site every other day by means of Boom Trucks.
 Redundant and waste materials are removed from the site once a week by means of cranes and trailer
trucks.
 Teams shall be identified to take ownership of the trash skips to ensure that wastes are not dumped
and left on the ground.
 Safety nets shall be cleaned of debris on a weekly basis.

LAYDOWN YARD
 Areas are demarcated and maintained for material storage.
 Continuous removal of excess and waste materials from the area.
 Properly segregation of materials is implemented.
 Daily housekeeping to ensure that waste materials and foodstuffs are not left behind that may cause
rodent infestation
 Temporary stores, offices and shelters are designed and constructed as per standard and safety
requirements.
 Flammable liquids are safely stored.

STAGING AREAS ON SITE


 Materials mobilized to the worksite are for 3 days use only.
 Solid barricade is provided around staging areas with specific entry and exit points.
 Materials are located inside the barricaded area.
 Safe walkway inside the barricaded area is established.
 Material to be staged does not pose a fall hazard should items be removed from stacks.

ACCESS TO WORK AREAS


 Solid barricade around work areas is provided and entry / exit points is identified.
 Signboard is provided to indicate the owner of the barricade.
 All red and white demarcation tape is removed from work areas upon completion of the work at the
end of each day.
 Work areas that present a risk or cannot be completed during the day but demarcated by tape, must be
secured with solid barricade before nightfall. This will ensure the safety of personnel working during
the night shift.
 Safety Managers to conduct, during afternoons, a site assessment to ensure all unnecessary
barricades have been removed from site.

COMPLIANCE TO PERMIT REQUIREMENTS


 Proper safe work planning was done, prior to compilation of the permit. Supervisors should walk the
area to ensure all prevailing conditions are considered and included in risk analysis
 100 % compliance to work permit requirements must be assured.
 Competent Fire Watch is designated for hot work activities, and remains on standby in the area after
completion of the activity.
 Adequate fire blankets are provided to ensure that no fireball or spark is visible beyond the blanketed
area.
 Under ground drainage systems or connections are properly covered to prevent any contact between
ignition sources and fumes in the drains.

USE OF PERSONAL PROTECTIVE EQUIPMENT


 Only United approved /hardhat liner (face cover) is allowed on site.
 Each worker on site is provided with earplugs, to be worn in areas where required.
 All personnel on site wear safety glasses, over specs or approved goggles. Personnel using
prescription glasses, without over specs or goggles will not be allowed access to the construction
site. On a second offense, his services will be terminated.

 Safety shoes are in good order to ensure proper grip.


 Use of full body harness with two shock-absorbing lanyards, where a 100% safe platform is not
provided, or where a fall hazard exists 1.8m above grade.

MAINTENANCE OF EQUIPMENT
 All equipment shall be inspected and color-coded as per United Procedure.
 Equipment mobilized to the site shall comply with project requirements.
 All electrical tools and equipment that are not double insulated must be equipped with a Ground Fault
Circuit Interrupter (GFCI).
 Air Compressors, pressurized hoses, etc., shall be installed with whiplash arrestors across hose
connections and couplings.

SAFETY PERSONNEL TO BE REMAIN VIGILANT AT ALL TIMES


 Where flammable liquids or gasses are suspected to be present, mobile phones, cigarette lighters,
matches or flashlights (that are not intrinsically safe) will not be allowed in the area. Contravening this
requirement will result in immediate dismissal from the site.
 Safety personnel must ensure 100 % compliance to all safety requirements
 In areas where LIVE SYSTEMS are present, or areas handed over to UNITED or in the event of
hazardous work being performed, the ratio of 15 personnel to one Safety Officer must be maintained at
all times.

ENFORCING DISCIPLINARY ACTION


 Employees will be immediately dismissed from the project for serious safety violations committed as
per United Safety Procedures.

HEALTH AND HYGIENE


 No foodstuff will be allowed on site or in the fabrication areas.
 Rodent control shall be enforced at mess halls, break shelters and trash bins/skips.

SAFETY AWARENESS SIGNS AND POSTERS


 Appropriate and relevant safety awareness signboards and information campaign and bulletins are
regularly posted in strategic locations at the worksite.
