1. General
As safety is of prime important in carrying out the work to achieve the targets. All the
agreed relevant safety documents and related formats as required by contract will be
followed.

2. Safety Training
All persons who work for this contract must have a safety orientation arranged by clients
prior to working and also to be briefed on daily Tool Box Meeting for each specific job
prior to working on that day.

3. Safety Meeting
Besides attending the safety meeting as requested by clients, SPM will also set up any
safety meeting whenever necessary not limited to the following meetings.

3.1) Daily Tool Box Meeting


This meeting will be held in everyday, and normally is set on 07:45 hrs. This will be
attended by all the members involving in the work. Safety precautions, accidental
prevention, using of protective tools, etc. will be reviewed/discussed concerning the
jobs to be performed on that day.

3.2) Periodic safety meeting


SPM’s Safety Supervisor and/or Field Manager must attend the periodic meeting as
required by clients.

4. Safety Patrol
A Safety patrol shall be conducted everyday around the job site by the safety supervisor.
The supervisor shall inspect and check any violations of the safety rules, and take the
necessary action immediately. And safety patrol reports will be submitted to client’s Safety
Manager by/about 10:00 hrs. on every working day. Also all incidents must be reported
immediately and be followed by a written investigation report within 24 hrs. of the
occurrence.

Particular attention should be set up in identifying safety hazards or critical working


conditions due to lack of awareness by inexperienced workers.

Site safety records should be maintained e.g. workers’ safety records, equipment
registrations, records required by statutory, etc.

5. General Rules
5.1) Smoking
Smoking will only be allowed in designed & controlled areas. Smoking is prohibited
in vehicles and at the job site. All vehicles, mobile compressors, etc. must be fit with
spark arresters, always keeping the exhaust pipe in good condition.

5.2) Fire Extinguisher


Fire extinguishers shall be provided at working places such as building office,
smoking areas, and hot work areas. Minimum of one fire extinguisher shall be placed
at these areas. All personnel are advised not to use the water fire-hydrant unless for
fighting the fire or permission is granted from the permit issuing by authorized
personnel of the clients.

5.3) House Keeping


Good house keeping is essential to safety, it can only be achieved and maintained by
the consistent efforts of all personnel. Waste materials must be disposed on each day
to the designated areas. Working areas are kept clean and free from rubbish, drinks
and food must not be brought in the work sites. Walk-says are kept free of
obstructions, and spillage should be cleaned up without delay. All material,
equipment, and tools shall be organized or storage in designated premises.

6. First Aid
SPM field office will provide also necessary first aid medicine and facility. Whenever
accident happen, First Aid Procedure will be followed to rescue the patient. The patient
will have first aid at SPM and/or main contractor offices and after that he will be sent to
Map Ta Phut Hospital, depending on the severity of his injury.

7. Safety Equipment
7.1) General
All workers shall put on proper working clothes & equipped with the necessary
personal protective equipment such as safety helmet, shoes, spectacles, ear-plug, etc.
Special personal protective equipment or devices shall be furnished as required e.g.
cloth mask with exhaust blower in confined space, and their use should be strictly
enforced. However, for nozzle-man, special facemask be used for covering from the
head in lieu of conventional safety helmet.

7.2) Safety Belt


Workers in confined spaces, steep slopes, swinging scaffolds, structural steel, or at a
height above 2 metres, should be secured with safety belts. Independent life line shall
be provided where necessary.

8. Work at Elevated Places


Scaffolds, ramps, ladders, and platforms shall be provided by main contractor or owner for
all work to be done at elevated places at 2 metres above ground level. Unauthorized
personnel shall be prohibited from entering the construction area. Safety ropes and caution
signs like NO ENTRY and DANGER OVERHEAD shall be displayed during construction.
Scaffolds, ramps, and platforms shall be kept free from grease, mud, or any other materials.

Materials, tools, and equipment that may be displaced or fall shall be safety secured.

9. Materials Handling
In transporting materials, rubbish, and tools at high level; appropriate devices, such as a
transporting chute, bag, container, etc. that tied securely with a rope shall be used whenever
necessary.

During these operation, a foreman shall carefully keep watch, provide NO ENTRY and
DANGER OVERHEAD signs, and block off the related working areas.

Excess materials shall not be placed on the work floor. Proper housekeeping shall be
maintained at all times.

Materials for refractory linings will be transported to owner’s warehouse and/or containers
to the work fronts and/or to SPM’s temporary warehouse/shelter with due care by craned-
truck and/or pick-up with the assistance of owner’s warehouse forklift for loading.

Materials to be put at the above locations must be on pallets, free from water and direct
sunlight. At the work fronts, tent or canvas will be used for such protections.

Should identifications of materials are in conflict on some pallet/bath and/or whatever will
happen, it is a must to consult with owner immediately prior to using, even in doubt of lump
piece or aging or expiring of the materials after opening in each bag.

10. Electrical Work


10.1) General
All electrical work shall be performed by qualified and experienced workers.
Equipment shall be locked or secured to prevent unauthorized persons from operating
it.
Live parts of apparatus and wiring shall be effectively guarded to protect all workers
form contacting it.
All equipment and wiring shall be checked daily by the operator before
commencement of work.
Warning signs or posters, such as DANGER, NO ENTRY, shall be displayed at
dangerous places, such as substations, switch boxes, and overhead or underground
cables.
10.2) Circuit Breakers
Circuit breakers shall be used against electrical shock. Provision of this breaker on all
electrical equipment, such as conveyors, winches, grinders, and others similar
equipment is compulsory.

10.3) Switch Box


Temporary switch boxes shall be constructed of metal with a waterproof roof and
lockable door and ELCB to be install in distribution panel.

Switch boxes shall be installed adjacent to the working area and elevated at a height
of 80 cm, from the ground.
Ample numbers of switch boxes shall be provided with adequate capacity. All
switches and fuses shall be of the appropriate capacities. Overloading beyond the
rated capacity shall be prohibited. Switch boxes shall be grounded with vinyl-
insulated copper wire.
Usage of copper or steel wire as an alternative to the standard fuse is strictly
prohibited. Multiple connection of cables form one source is not allowed.
While inspecting or repairing work is being carried out, all switches shall be cut out
from the power source.

11. Construction Equipment


11.1) General
Prior to use of all construction machinery and equipment, all shall be inspected and
tested by competent and qualified professional engineers and confirmed to be in safe
operating condition. Inspection and tests shall be conducted at least once a month to
ensure a safe operation.
Any machinery or equipment found to be unsafe shall not be used until the unsafe
conditions have been corrected.
11.2) Safety Devices
All belts, gears, shafts, pulleys, sprockets, spindles, flywheels, or other reciprocating,
rotating, or moving parts of equipment shall be provided with protective guards to
prevent unnecessary contact, which may be otherwise dangerous.
All drums on load-hoisting equipment shall be equipped with proper ratchets or other
positive locking devices.
Stoppers shall be installed on the hook of the crane to prevent the rope form slipping
off.
All pressurized equipment and system confirmed shall be equipped with approved
safety or relief valves and pressure gauges.
11.3) Pressurized equipment
Any pressurized equipment or system confirmed to be unsafe shall be tagged OUT OF
SERVICE, DO NOT USE. Use of such equipment shall be prohibited until the unsafe
condition is corrected.
Pressurized equipment shall be operated and maintained only by qualified and
authorized personnel.
It is advisable that the setting of the safety valve be limited at 110 percent of the
working pressure. The discharge from safety valves, relief valves, and blow-off shall
be directed away from personnel.
Safety appliances and devices shall not be removed unnecessary.

Compressors and related equipment shall be placed to provided safe access to


workers.
All air compressor shall automatically stop before the discharge pressure exceeds the
maximum allowable working pressure.
Stop valve shall be installed at each outlet for the air hoses. The stop valve on the air
hose shall be closed when a tool is being changed or repair work is in progress.
Drain valve on the air receiver shall be provided & opened to drain out the oil before
commencement of work.

12. Hand Tools


12.1) General
All hand tools shall be kept & maintained in a satisfactory condition and used only
for the purpose of which they are designed. Prior to usage, all power tools shall be
inspected and tested to ensure safe operating conditions. Periodic inspections on
these tools shall be maintained.
Defective tools shall not be used.
When work is being carried out at an elevated position, tools not in use shall be
secured or placed in holders.
Hurling tools or materials form one location to another, form one worker to another,
or dropping them to lower levels is strictly prohibited.
Power tools shall be operated only by authorized personnel.
Flexible rubber cable shall be used for all portable electrical tools.
Cotton gloves shall be prohibited during drilling or grinding work.
12.2) Grinding Tools
Cracked or damaged grinding wheels should be prohibited.
12.3) Pneumatic Tools
Compressed air shall be exhausted from the line before disconnecting of tools.
Operating pressure on the air hose shall not exceed the rated pressure specified by the
manufacturer. Defective hoses should be replaced.
Hoses should not be left on ladders, steps, scaffolds, or walkways.
The use of compressed air for blowing off dirt from hands, face, or clothing are
prohibited.

13. Working in confined space


13.1) Checking that the space has been free of gas, vapour, etc. and has been released by
area owner in writing for accessing.
13.2) Making sure and arranging sufficient ventilation and exhaust openings and installing
blowers and/or exhaust fans at the proposed locations.
13.3) Consulting with area owner in case openings are insufficient, this may require
respirator to suit the working conditions e.g. materials to be applied/lined.

13.4) Providing ramps, scaffolding, and platform in case working area is higher than 2.00
metres. Side protections e.g. rope or rail guard must be provided together with ladder
or stairs. Necessary protection supports must be provided under the
scaffolding column/legs not to damage the floor.
13.5) Providing sufficient lighting with specified voltage protected bulb.
13.6) Providing permit tag for assessment clearly shown at the entrance, and seeing the
allowing time period mentioned on the permit prior to enter for working.
13.7) The crafts who will enter for working must have all safety protections dressed up e.g.
safety helmet, shoe, goggles, belt, ear plug, mask/respirator (if work with toxic
substance), life line, etc.
13.8) Checking the space randomly with Oxygen detector ensuring that Oxygen content is
more than 16% prior to working; and should be checked for every 2 hours.
13.9) Fire extinguisher should be provided if working with solvent or similar material.
13.10) Water an/or sloppy substance must be drained or cleaned out from the working
floor.
13.11) Providing telecom set between the internal working group and the one watching at
the entrance and/or machine operator, if necessary.

14. Sanitary
For hygenic and by regulations use only the provided sanitary servicing place and keep
thing in tidy and clean after using such services.

1. SCOPE

This manual covers the accident prevention rules and safety programs that will apply
To al personnel engaged in construction work. The primary purpose of this manual is
to prevent accidents which may injure personnel or damage property of the clients.

Any local government law, municipal ordinance or rules and regulations prescribing
safety for construction work shall prevail over this manual.

2. ORGANIZATION

2.1. General

The safety programs covered in this safety manual shall be carried out by the safety
organization shown in company organization chart.

2.2. Safety Committee

SPM shall organize a safety committee consisting of field manager and safety
supervisor and the necessary supervisors. These will be coordinated with office
Safety Staff. The purposes of the safety committee will be as follows.

(2.2.1) To carry out safety programs.


(2.2.2) To instruct and direct safety programs.
(2.2.3) To perform work safely.

2.3. Safety Meeting

A safety meeting presided over by owner or main contractor of field manager shall
be held weekly. This meeting shall be attended by all members of the safety
committee for site work and yearly for office.

The safety meeting shall be held immediately after an accident, a safety patrol is
made to make the meeting effective. The findings of the safety patrol, review of
work plans, etc., shall be discussed at the safety meeting.

2.4. Safety Patrol

A safety patrol shall be conducted around the jobsite by the supervisors every
morning. The safety patrol shall inspect and check any violations against the stated
safety rules.

Particular attention shall be paid to disclosing hazards of critical working conditions


due to a lock of awareness by inexperienced workers. A file of the safety patrol
reports by adapting the format given Appendix 2 to meet job requirements, and shall
be kept at the jobsite during the entire construction period.

3. RESPONSIBILITY

3.1. Safety Coordinator

The Technical Manager will act as Safety Coordinator of the company with safety
committee at sites to carry out the work safely during the construction period. The
responsibility of the safety coordinator is to observe that all company’s employees
abide by this manual. Any violations found shall be recorded and counter measures
be taken.

3.2. Field Manager

Field manager shall be responsible for safety, including fire prevention, during the
construction period. He shall organize and preside over the safety committee to
ensure that the work is executed safely by all personnel.

3.3. Safety Super visor

Safety supervisor shall assist the field manager and conduct the safety programs
under the direction of the field manager, The safety Supervisor shall check each
work plan or procedure for safety before the commencement of work. He shall
give his opinion or concept to the field supervisors.

3.4. Field Supervisor

Field supervisors shall check and confirm the work procedure regarding safety
before the commencement of work. They shall supervise the work concerning
safety, such as safety programs specified in this manual. If violations or unsafe
work conditions are detected, the field supervisors shall immediately advise or
instruct to the workers to correct them. Work shall be stopped until violations
or unsafe working conditions are corrected.

Field supervisors shall always work in close contact with the safety supervisor to
carry out the work safely. For work involving hazards of a fire, explosion, etc.,
the field supervisors shall direct the work throughout its performance

4. GENERAL RULES

4.1. Identification

All employees working on the jobsite shall wear a helmet with SPM logo sticker or
badge on their outer garment at all times within the jobsite. This badge shall show
the registered number and category or each worker.

4.2. Traffic Rules

(4.2.1) Driver’s License


Motorized equipment shall be operated only by a licensed driver.

(4.2.2.) Parking
All vehicles shall be parked at the designated area except vehicles in use
for construction work. Motor vehicles and other motor vehicles and other
mobile equipment shall not be parked adjacent to fire hydrants, fire
fighting equipment, building exits, walkways, etc. In principle, all private
cars shall be forbidden from the construction area.

(4.2.3.) Warning Signs


All workers shall obey the street stop and warning signs. Materials
hanging over the end or sides of a vehicle shall be marked with a red
flag.

(4.2.4) Speed Limit


All drivers shall obey the speed limit.

4.3 Clean Up

All materials, equipment, and apparatus should be stored neatly in the designated
area. All scraps, waste materials, and oily rags should be collected and stored q
separately, at least once daily.

4.4. Posters and Signs

Posters and signs shall be adopted as visual aids for accident and fire prevention.
Posters shall be written in English and Thai, They shall be conspicuously
displayed, These posters and sings shall not be removed or replaced.

4.5. Fire Prevention

4.5.1) Hot Work Permit


Fire hazardous areas requiring hot work permits shall be designated by the
client’s safety engineer, Hot work at the designated hazardous area shall be
approved by the client’s safety engineer, Clients’ application format will be
used for filling in all the information required for hot work.

4.5.2) Smoking
Smoking will be allowed only in the designated area under controlled
conditions. Smoking is prohibited in vehicles at the jobsite.

4.5.3) Fire Wall


A fire wall shall be constructed between the existing plant and the
construction area, where

Flame arrestor shall be installed where necessary and be checked by
client’s safety engineer.

4.5.5) Fire Extinguisher


Fire extinguishers shall be provided where the used of fire is permitted,
such as in buildings, offices, smoking areas, and hot work areas. At least
one fire extinguisher shall be placed at each of those areas.

4.6 Accidents

(4.6.1) Injuries
At the jobsite, first aid facilities meet the requirements of the local laws
and regulations for all persons engaged in the construction work. When a
person is critically injured, he shall be taken to the designated hospital
immediately after emergency medical treatment.

(4.6.2) Fire
In case of fire, the discoverer shall immediately notify the Client’s fire
department and field office on any telephone, Persons working adjacent to
the site of the fire shall immediately try to put out the fire with fire
extinguishers if the fire is small. In case of fire in the existing plant, all
construction work at the jobsite shall be stopped, and all construction
equipment and electrical switches shall be turned off. All personnel shall
evacuate immediately.

(4.6.3) Accident Follow Up


Safety supervisor shall write an accident report (Appendix 4) for all
accidents involving personnel regardless of the severity of the accident and
submit it to SPM and/or client’s construction manager. Accident statistics
will be compiled monthly. All accidents shall be investigated immediately
by the safety committee to clarify the cause and take corrective measures
against recurrence of similar accidents safety supervisor shall inform all
workers of corrective measures.

4.7 Excavating
An excavation permit shall be obtained for any excavations to be made adjacent to
the existing plant before commencing the work to avoid damage to underground
power lines, piping, etc. Warning signs, such as rope painted yellow and black, shall
be placed where underground power lines and piping exist.

5. SAFETY EQUIPMENT

5.1 General

All workers shall wear suitable working clothes, Every worker shall wear a safety
hat on the jobsite, Personal protective equipment or devices shall be furnished as
required, and their use shall be enforced.

5.2 Sight and Face Protection

Workers engaged in gas welding, cutting, and brazing shall be required to wear
goggles or face masks when necessary to protect themselves from injuries.

Workers engaged in electric welding, cutting or other similar operations shall be


required to wear helmets or shields with filters of appropriate shade and gloves
when necessary. Workers engaged in grinding or similar work shall be required to
wear goggles with safety filters for sight protection or face masks giving equal
protection.

5.3 Safety Belt

Workers in hoppers, bins, or confined spaces of on steep slopes, swinging scaffolds,


structural steel, or unstable work at an elevation 2 meters or more above the ground
shall be secured by safety belts and independent life lines.

6. WORK AT ELEVATED PLACES

6.1 General

Scaffolds, ramps, ladders, and platforms shall be provided for all work to be done
2 meters or more above the ground.

Erection, replacing, and dismantling of scaffolds, ramps, and platforms shall be


directed by the field supervisors.

Unauthorized persons shall be prohibited from entering the construction area


scaffoldings, ramps, and platforms. Safety ropes and caution signs like No ENTRY
and DANGER OVERHEAD shall be displayed during construction.

Scaffolds, ramps, and platforms shall be kept free from grease, mud, or any other
material or equipment.

The maximum allowable load and name of person responsible for scaffolds, ramp,
and platforms shall be displayed.

Wooden materials used for construction of scaffolds, ramps, platforms, and runways
shall be of good quality. Synthetic or natural fiber ropes or other similar flexible
materials may not be used as guard rails.

Safety nets shall be provided where scaffolds are infusible or unavailable, such as
when assembling pipe rack, stack and structure. In such cases, the workers shall
always use safety belts.

All openings and ends of platforms located 2 meters or more above the ground shall
be barricaded or covered.

All scaffolds, ramp, and platforms shall be inspected regularly every month, and any
damaged or unsafe portions shall be repaired or replaced.

Work at high elevations without a safety belt and safety net shall be prohibited.

6.2 Metal-Tube Scaffolding


The ground on which the uprights are to be erected shall be firmly compacted. If the
ground in uneven, adjustable metal base plates shall be used.

The spacing of the uprights shall not exceed 1.8 meters longitudinally and 1.5
meters in the span direction. Joints of the uprights shall be in a line but staggered.

The first row of ledgers from the ground shall not exceed 2 meters from the ground
level. The vertical spacing between ledgers located above the second row of ledgers
shall be from 1.5 to 1.6 meters.

The spacing of transoms shall not exceed 1.5 meters. To strengthen the scaffolding,
a sufficient number of longitudinal braces shall be provided diagonally at an angle
of about 45 degrees on the façade of the scaffold and secured rigidly.

Metal fittings shall be secured and tightened with proper tools.

6.3 System Scaffolding


System scaffolding shall be erected in accordance with the manufacturer’s
specification

The scaffold height generally shall not exceed 45 meters, but system scaffolding
which exceeds 45 meters shall be designed for the strength. The height of the
upright frame shall not exceed 2 meters, and the spacing shall not exceed 1.8
meters. Upright frames shall be tied to cross braces and ledger frames.

Braces shall be fixed by pins or screws to prevent loosening.

The spacing of ties or stays for other walls shall not exceed 5 meters vertically and
5.5 meters horizontally.

6.4. Tank Scaffolding


Structural steel. Steel straps, and steel bars used for brackets, hooks, and hangers
shall be free from any damage or corrosion.

Guard rails shall be wire not less than 9 mm. in diameter or steel pipe no less than
13 mm. in diameter. Nylon rope shall not be used.

6.4 Platform

The width of each platform and ramp shall not be less than 40 centimeters. Planks
used for a platform of ramp shall be uniform in thickness (minimum 3.5
centimeters) and spacing of each plank shall be not more than 3 centimeters. Width
of each plank used for platform and ramp shall not be less than 20 centimeters.
Planks shall be supported on at least two frame connectors and tied firmly to
prevent excessive springing, deflection, loosening, or displacement. Each end of a
plank shall extend 10 centimeters beyond the frame connector. Joints of planks shall
lap 20 centimeters with each other, and the center of the lap shall be on the center
line of the frame connector.

6.5 Safety Net


Work requiring a safety net shall not be undertaken until the net is in place. Safety
nets shall extend 2 meters beyond the edge of the work area when workers are
exposed to the hazard of falling

Safety net shall be suspended by chains or links with hooks at least at the four
corners. They shall be lapped one meter at the edges instead of being butt jointed.

Safety nets shall be inspected daily for damage caused by abrasion, chemicals, or
heat. Any damaged portions shall be mended. Debris shall be removed from safety
nets at least daily, and combustible materials removed before welding, cutting, or
doing work which emits sparks, slag, or other ignition sources.

6.7 Ramps and Ladders


Ramps and ladders shall be provided to secure safe passage for workers required to
work at a height or depth of more than 1.5 meters.

As a rule, the slope of ramps shall be limited to 30 degrees. For slopes of more than
30 degrees, rungs and guard rails shall be provided. The rungs shall be spaced 30
centimeters apart and nailed firmly.

One Platform shall be provided for every 7 meters in height.

All ladders, ramps, runways, ant the surrounding areas shall be free of materials,
machined, and obstructions at all times.

6.8 Material Handling


Materials, rubbish, and tools shall not be thrown from upper levels to lower levels
the ground.

When lowering or moving materials on the ground, suitable devices, such as a


chute, bag, container with a rope, or a device tied securely with rope, shall be used.

During the work, the foreman shall carefully keep watch, provide NO ENTRY and
DANGER OVERHEAD signs, and rope off the related work area for safety.

Excess materials shall not be placed on the work floor or runway.


Materials, tools, and equipment which may be displaced or fall shall be secured
adequately.

7. WORKING IN CONFINED SPACE

7.1 General
These general basic safety tools/facilities are same as other place but safety
precautions are more serious than working out-side. Many points have to be taken
into consideration prior to working in confined spaces.

7.2 Working Procedure

(7.2.1) Checking that the space has been free of gas, vapour, etc. and has been
released by owner/main-contractor in writing for accessing.

(7.2.2) Making sure and arranging sufficient ventilation and exhaust openings and
installing blowers and/or exhaust fans at the proposed locations.

(7.2.3) Consulting with owner/main-contractor in case openings are insufficient,


this may require respirator to suit the working conditions e.g. materials to
be applied/lined.

(7.2.4) Asking responsible party to provide ramps, scaffolding, and platforms in


as working area is higher than 2.00 metres. Side protections e.g. rope or
rail guard must be provided together with ladder or stairs. Necessary
protection supports must be provided under the scaffolding columns/legs
not to damage the floor.

(7.2.5) Providing sufficient lighting during working.

(7.2.6) Providing permit tag for assessment clearly shown at the entrance, and
seeing the allowing time period mentioned on the permit prior to enter for
working.

(7.2.7) The crafts who will enter for working must have all safety protections
dressed up. e.g. safety helmet, shoe, goggles, belt, ear ;lug, respirator (if
work with toxic substance), life line, etc.

(7.2.8) Asking our or owner’s safety supervisor to check the working space
randomly with Oxygen detector ensuring that Oxygen content is about
16% prior to working; and should be checked for every 2 hours.

(7.2.9) Fire extinguisher should be provided if working with solvent or similar


material.

(7.2.10) Water an/or sloppy substance must be drained or cleaned out from the
working floor.

(7.2.11) Providing telecom set between the internal working group and the one
watching at the entrance and/or machine operator, if necessary.

(7.2.12) Providing a fixture for attaching bade or gate passes of the personnel
going into the confined space. This can be checked the numbers of persons
working in and/or everyone is exit safety since the exit personnel will take
bade their badges after exiting.

8. ELECTRICAL WORK
8.1 General
All electrical work shall be performed by qualified and experienced workers.
equipment shall be locked or secured to prevent starting by unauthorized persons.

Live parts of apparatus and wiring shall be effectively guarded to protect all workers
or objects from coming into contact.

All equipment and wiring shall be checked daily by the operator before starting
work. All electrical circuits shall be grounded.

Warning signs or posters, such as DANGER, NO ENTRY, shall be displayed at


dangerous places, such as substations, switch boxes, and overhead or underground
cables.

8.2 Transformer Bank


Transformer banks or high voltage equipment shall be barricaded with a fence.
The entrance shall be locked.

For transformer banks, warning signs of DANGER, NO ENTRY and NO


WATER SPRAYING shall be posted. Posters shall show the name of the
supervisor or maintenance engineer. Primary and secondary voltage and
capacity also be displayed.

8.3 Circuit Breakers


Circuit breakers shall be provided for all electrical equipment, such as conveyors,
winches, pumps, grinders, and similar equipment, to prevent workers from being
injured by electric shock. Automatic leakage breakers install in switch boxes are
preferable.

8.4 Switch Box


Temporary switch boxes shall be constructed of metal with a waterproof roof and
door which can be locked.
Switch boxes shall be installed adjacent to the work area to a height of 80
centimeters or more from the ground or working floor.
An ample number of switch boxes shall be provided with adequate capacity.
Switches and fuses shall be of proper capacity for the circuit protected.
Overloading beyond the rated capacity shall be prohibited.

The name of the person responsible for inspection and maintenance of the switch
box shall be marked on every switch box. Name or number of equipment to be
served by each switch shall be tagged on the switch.

Switch boxes shall be grounded with vinyl-insulated copper wire.


Using copper or steel wire instead of a fuse shall strictly be prohibited. Multiple
connection of cables from one switch shall be prohibited.

When inspecting or repairing, switches shall be cut out and the switch box locked.

8.5 Welding Machine


Before welding machines are used, insulation shall be tested and certified to be in
safe operating condition. Automatic anti-electric shock devices shall be provide for
all welding machines and tested every day before work is started.

All exposed terminals shall be covered safely with insulation tape.

A tag printed with the name of the responsible person and company providing the
welding machine shall be attached to the machine.

Welding machines shall be installed 30 centimeters or more away from the switch
box.

8.6 Movable Wiring


Wiring shall be cab-tyre cable having a dielectric strength of not less than the rated
voltage of 600 volts. Cable having ample capacity to the load shall be use.

Cable with any external damage shall not be used.

All cable shall be installed away from any steel material, such as wire rope, steel-
frame scaffold, etc.

8.7 Grounding
To prevent short circuits or electric shocks, special precautions, such as grounding,
shall be taken for wiring work where metal scaffolds or steel structures are erected.

Grounding shall be insured by connecting the wire to an earth rod buried firmly in
the ground. Brass or steel bolts and nuts shall be used for grounding terminals of all
electrical equipment.

9. ERECTION WORK
9.1 General
The erection work plan and procedure shall be checked thoroughly by field
manger and supervisors.

Warning signs of NO ENTRY and safety ropes shall be provided at the instruction
of supervisor.

The strength of the road and any obstacles on the route of the crane shall be
checked. The road shall be reinforced and obstacles removed, where required.

The strength of the ground where the crane is be placed shall be examined. If
necessary, reinforcements, such as installation of steel plate, shall be made.

Only authorized persons with a license shall be permitted to operate the crane or
do Slinging for lifting equipment.

The foreman and signal man shall be assigned for each erection project under an
operation system established for the work. They shall stand where they can
observe the loading operation and be clearly visible to the crane operator during

the crane work. A uniform signal system shall be used with flags, transceivers, and
whistles for signaling.

The capacity of the crane to be used for erection shall be determined after careful
consideration.

Total lifting weights and the center of gravity of equipment to be erected shall be
carefully rechecked before commencing erection work.

Before starting the work, the condition and functioning of the brakes, limit switch,
over-roll preventing devices, wire rope, and lifting devices shall be checked and
inspected, and the crane shall be operated on a trial basis without a load. As a
preliminary check, the equipment shall be lifted and held 10 centimeters from the
ground. In this state, all elements shall be checked and inspected to see if they are
functioning properly. If risky conditions are detected, the lifting work shall stop
immediately.

Lifting work shall not be carried out during bad weather, such as strong winds
or rain.
All crane and winches shall be tested and inspected regularly each month. Cranes
or winches shall be locked or braked when not in operation.

9.2 Erection by Crane


Lifting work shall begin only after confirming by preliminary check that the crane
is set up satisfactorily.

The crane shall be secured horizontally, and steel plates or square timbers shall be
placed under the outriggers firmly, then secured with knock pins after being set in
position.

The operator shall not leave the crane or winch during lifting work.

The inclined angle of the boom during operation shall not exceed the range of 30
to 80 degrees unless otherwise specified for the machinery. When using the job,
its length must be minimized. An angle indicator shall be provided for the crane
operator to visually check the boom angle. Crane booms shall be lowered to the
ground level, and the hook shall be secured to the specified position when the
crane is not in use.

Crane shall never be loaded in excess of the manufacturer’s stipulated rating.


Lifting load for each crane shall be controlled within 90 percent of the maximum
lifting load. The lifting load shall include the dead load of lifting hook, rope, etc.
During the lifting work, the operation shall be carefully supervised to prevent
hasty lifting, prolonged suspension, and lifting beyond the limit. Abrupt lifting
and stopping shall be prohibited.

Simultaneous rotation and lifting or rotation and boom movement shall be


prohibited.

The boom shall be slowly rotated so as not to produce centrifugal forces on


equipment or materials being lifted.

Workers shall be forbidden to ride on lifted equipment or material when lifting or


swinging is taking place.

The crane’s wire rope shall be rolled up entirely after the work is finished.

9.3 Winch Work


The winch shall be installed on firm ground with a good view for the work. The
area where the winch is installed shall have maple space for inspection and
maintenance and be kept clean at all times.

The winch drum diameter shall be 20 times the diameter of the wire rope. The end
of the wire shall be fixed securely to the winding drum. Wire ropes shall be wound
regularly the winding drum. Warning markers shall be attached to the wire rope to
prevent over winding.

The winch shall be installed securely to prevent floating, slipping, and vibrating.
Foot brakes shall be provided with adequate spacing from the ground to permit the
brakes to be operated freely. Gears shall be provided with a cover. The operating
winch shall be located properly for convenient operation. Winch motors shall be
grounded.

Winch work shall be performed under the signal of the signal man. When winch
work is finished or suspended for a while the power shall be switched off. The
winch operator shall not leave his position during winch operations.

9.4 Rigging Work


Rigging work shall be performed under the direction of an authorized foreman.
The lifting weight shall be the total weight of the equipment of materials, crate,
and lifting tackle.

Ropes shall be of good quality, free from rust, corrosion, and deformation. Wire
rope shall be replaced when either of the following critical conditions is observed :
(1) Quantity of broken wires exceeds 10 percent of the total in one strand.
(2) Reduction in diameter exceeds 7 percent of the original.
Materials shall be lifted by at least 2 ropes, and the sling angle shall not exceed
60 degrees.

The coefficient of safety for the main wire an other rigging shall be 6 or more
when in use.

Padding shall be provided at sharp edges of materials to be lifted to prevent the


ropes and slings from being damaged or cut.

A guy rope shall be provided for extra-long or unstable special materials to be


lifted.

The hook shall be placed above the center of gravity of the material to be lifted.
When the center of gravity is in the upper half or not in the middle of the material,
Special precautions shall be taken to prevent swinging, slipping, or falling.

Working under lifted materials or riding on lifted materials shall be prohibited.

Working ropes and slings shall be removed after confirming the lifted material is
stable.

10. CONSTRUCTION EQUIPMENT

10.1 General
Prior to use, all construction machinery and mechanized equipment shall be inspected
and tested by a competent mechanical engineer and confirmed to be in safe operation
condition. Inspection and tests shall be conducted at least once a month to assure safe
operation. Test and inspection records shall be kept in the files.

Any machinery or equipment found to be unsafe shall not be used until the unsafe
conditions have been corrected. All repairs on machinery and equipment shall be
executed at a location designated for this purpose.

Maintenance procedures recommended by the manufacturer shall be followed.

Machinery and equipment shall be operated only by designated personnel.


Machinery or equipment requiring a licensed operator shall not be permitted to be
operated without such an operator. Machinery and equipment shall be locked or
secured to prevent their being started by unauthorized persons.

Stationary machinery shall be placed on a firm foundation and secured before being
operated.
Mobile machinery shall have a braking system.

The engine of the machinery shall be stopped during fueling.

Bulldozed scraper blades and loader buckets shall be lowered when not in use.

On all machinery and equipment, the name of the responsible person, operator,
and company shall be posted, painted, or tagged.

Machinery shall be used only for the purpose for which it was designed.

10.2 Safety Devices


All belts, gears, shafts, pulleys, sprockets, spindles, flywheels, or other reciprocation,
rotating, or moving parts of equipment shall be provided with protective guards to
prevent contact by personnel.

All drum on load-hoisting equipment shall be equipped with proper ratchets or other
positive locking devices.

10.3 Pressurized Equipment


Any pressurized equipment or systems confirmed to be unsafe by tagged OUT OF
SERVICE, DO NOT USE. Use of such equipment shall be prohibited until the unsafe
condition is corrected.

Pressurized equipment shall be operated and maintained only by qualified and


authorized personnel.

It is advisable that the safety valve setting not exceed 110 percent of the working
pressure. The discharge from safety valves, relief valves, and blowouts shall be
directed away from personnel.
Safety appliances or devices shall not by removed, except to make repair.
Compressors and related equipment shall be placed to provide safe access to workers
for equipment operation, maintenance, and repairs.

All air compressors shall automatically stop before the discharge pressure exceeds
the maximum working pressure allowable on the weakest portion of the system.

A stop valve shall be installed at each outlet for the air hoses. The stop valve on the
air hose shall be closed when a tool is being changed or repair work is in progress.

A drain valve on the air receiver shall be opened to drain the oil before commencing
work.

11. HAND TOOLS

11.1 General
All hank tools shall be kept in satisfactory condition and used only for the purpose
for which they are designed. Prior to use, power tools shall be inspected and tested
to ensure safe operating condition. Periodic inspections shall by made to assure safe
operating conditions.

Defective tools shall not be used.

When work is being carried out at an elevated position, tools not in use shall be
secured or placed in holders.

Hurling tools or materials from one location to another, from on e worker to another,
or dropping them to lower levels shall be prohibited.

Only non-sparking tools shall be used in locations where hazardous vapors exist
which may initiate a fire or explosion.

Power tools shall be operated only by designated personnel.

Flexible rubber cable shall be used for all portable electric tools. Waterproof
connectors shall be used for cable connections.

Wearing gloves shall be prohibited during drilling or grinding work.

Electric hand tools shall be grounded.

11.2 Grinding Tools


Use of cracked of damaged exhausted from the line before disconnecting tools.

11.3 Pneumatic Tools


Compressed air shall be exhausted from the line before disconnecting tools form
the line.

Air hoses shall be pressure rate by the manufacturer, and this pressure shall not be
exceeded. Defective hoses shall not be used.

Houses shall not be left on ladders, steps, scaffolds, or walkways.

The use of compressed air for blowing dirt form the hands, face, or clothing is
prohibited.

12. WELDING AND CUTTING

12.1 General
All welding and cutting apparatus shall be inspected daily. Defective apparatus and
equipment shall be replaced or repaired.

Combustible materials, equipment such as gas cylinders, rubber hoses, and debris
shall be removed or shielded from welding and cutting.

Welding or cutting work at elevated places shall be conducted only under safe
conditions. A safety belt shall be worn.

Welding or cutting in an area where flammable vapor may be present shall be


carried out carefully after, under the supervision of client’s safety engineer and/or
company’s safety supervisor, confirming with a gas detector that the area is safe.

When welding or cutting in confined areas for a long time, the space shall be well
Ventilated. During the work, the concentration of oxygen in the air must be greater
than 18 percent. When working in a confined area, the workers should at least be
paired to avoid accidents.

12.2 Welding Work


Welding machines shall be placed 15 centimeters above the ground to keep them
dry.

Welding machines shall be placed level and secured with a suitable wedge to keep
them in a horizontal position.

All electric welding machines shall be effectively grounded. The ground lead for the
welding machines shall be mechanically strong and electrically adequate for the
service required.

Approved connectors shall be used to connect the welding cable between the welding
machine and electrode holder.

Only electrode holders that have passed the inspection shall be used.

Whenever welding work is suspended or the welder leaves the construction area, the
switch shall be turned off and the welding rod disconnected from the holder.

12.3 Gas Work

Cylinders shall be stored in a well ventilated location and shielded from direct
sunlight with steel plates or incombustible canvas.

It is preferred to have the cylinders in service secured in portable racks or hand


carts.

Cylinders transported by crane, hoist, or derrick shall be loaded on cradles, nets,


or skid pans, but never in slings or chains or by magnets.

Cylinders caps shall be in place when cylinders are in storage, in transit, and
whenever the regulator is not in place.

Acetylene cylinders shall be in an upright position during use and secured to


prevent displacement.

Oxygen cylinders and fittings shall be kept away from oil or grease and not handled
by oily hands or gloves.

All cylinders shall be tagged FULL or EMPTY, and empty cylinders shall be
returned immediately to the storage area.

All connections between hoses, hose and torch, and hose and regulator shall be
tightened rigidly with hose bands.

Pressure gauges with cracked glass or damaged regulators shall be renewed.

Before starting work, a leakage test shall be made at all connections with a soapy
solution.

The torch valve shall be closed, and the gas supply shall be shut off whenever work
is suspended.

When work is completed, the cylinder’s block valve shall be closed, and regulators
and hoses shall be removed and put in the tool box.

1. Scope
The principal role of quality control is to see if the construction work complies with the
drawings, specifications, and project standards. Field inspection is conducted to evaluate
compliance with the project quality requirements. Field inspection covers not only the
work performed by the field craftsmen, but also the condition of the materials incorporated
into the work.

2. Responsibilities
Field inspection will be conducted by SPM’s own forces or those of inspection agencies.
The manage of the Quality Section assigns field inspector to monitor field activities. The
field inspector will document his inspection efforts. Inspection is usually done in two
ways:
By frequent inspection during the work.
By final proof or testing of the completed work.
A chief field inspector will be assigned is more working scope. He has the overall
responsibility for field inspection activities and coordinates the project inspection effort to
ensure the desired results. He will represent the company but under the capacity
construction or field manager in all questions of field inspection. He will verify the
inspection documentation and test results and issue field inspection reports to the client
and project organization through the construction or field manager. He will also report to
the Quality Section Chief in Bangkok Office. The term “inspector” mean “chief field
inspector or supervisor”.
Normally, function of the inspector will be defined in a work execution plan to cover the
inspection scope requiring by clients.

1. Scope of Work
Inspection work for field construction shall include, but not be limited to, those items
listed in Appendix 2, Field Inspection Items. Appendix 2 also classifies inspection items
into those inspected by SPM. Only, those for which the clients notified in the inspection
schedule, those witnessed by the clients, and those for which results are reported to the
clients.

2. General Policies and Procedures


The inspector shall initiate field inspection general policies and procedures referring to
process requirements, construction workers’ capabilities, and other jobsite conditions for
review and approval by the construction manager.

3. Detailed Procedures
The inspector shall document detailed procedures, both internal matters and for matters
relating to the client, including schedules, forms for recording inspection results, and
reporting procedures by following contractual formats.

4. General Pre-Inspection Meeting with the Client


The inspector shall hold a general pre-inspection meeting with the clients before beginning
any inspection activities. The inspector will present SPM’s proposed procedures for
inspection and testing as well as a list of test items which to be required by contract or
SPM recommends for witnessing by clients.

5. Individual Inspection and Test Procedures


The inspector shall establish individual inspection and test procedures for a particular
category of work prepared by the field supervisor or engineer in charged, including
choosing those items to recommend clients to witness.

6. General Pre-Inspection Meeting with Third Party Laboratory


To present SPM’s inspection and test procedures, the inspector shall initiate and conduct a
pre-inspection meeting with third party laboratory and the field supervisors in charged
prior to commencing construction work. In this meeting, SPM shall clarify all of the
requirements and criteria for the inspection and ensure that they are clearly understood.
All parties shall confirm that the procedures reviewed, including corresponding drawings
and specifications, will result in the desired quality and workmanship.

7. Personnel and Procedure Qualification


When construction procedures or personnels who carry out construction activities must be
qualified in accordance with applicable regulations, codes, specifications, or procedures,
the inspector shall witness the qualification tests and determine the validity of the results
shall be limited to areas which affect the quality of the work.

8. Check of Inspection, Test, and Measuring Equipment


Prior to every inspection, the inspector shall verify the validity of adjustment and
calibration of inspection, test, and measuring equipment. During verification, records of
previous calibrations shall be referred to by the inspector. When inspection, test, or
measuring equipment is found to be incorrectly calibrated, he shall evaluate the validity of
previous inspections or test results. Where necessary, the inspector shall repeat the
original inspections of tests using properly calibrated equipment.

9. Individual Pre-Inspection Meeting with the Client


The inspector shall hold individual pre-inspection meetings with the clients before starting
a particular category of work. Detailed procedures will be discussed, and the list of test
items to be witnessed will be given to the clients.

10. Daily Schedule of Tests and Inspections


The inspector shall arrange the daily schedule of tests and inspections, subject to the
schedule of client’s inspector, and coordinate the field supervisors’ activities with this
schedule.

11. Application for Inspection


The inspector shall submit the application for inspection of specified items to client in
advance of the inspection date (usually every morning or every evening to discuss work
for the following day). Appendix 3 is an application form in case contractual format is
contractual format is unavailable.

12. Client’s Witness


Whenever client is to witness an inspection or test, the inspector shall attend the
inspection. After completion of the witnessed test, he should record the test results and
obtain the signature of acceptance from client’s inspector on the inspection form. A
written report of the results should be submitted to clients.

13. Regular Inspection Meetings with the Client


The inspector shall hold regular inspection meetings with the client to exchange comments
on inspection matters. In these meetings, all inspection requirements and criteria shall be
clarified by both the client and Engineer (consultant). The field supervisor may
accompany the inspector to these meetings. They may hold joint meetings for both
construction and inspection activities with the clients, if desirable. Problems should be
presented with vendor-specialists and/or SPM’s proposed solutions or recommendations
for further study.

14. Relations with the Client


As a representative of SPM, the inspector shall use every opportunity to convey SPM’s
ideas, comments, or suggestions for inspection procedures to clients. The inspector, in
turn, will solicit and consider the client’s opinion on these matters. For conserving good
relations with clients, the inspector shall work with the clients to mutually clarify any
ambiguous, dubious, conflicting, inconsistent, or questionable matters in the specification
requirements or inspection criteria. Close liaison with client also is needed to mutually
study and clarify any problematical, delicate, or intricate matters in inspection
requirements, procedures, or practice. Reporting results of inspections and tests should
demonstrate SPM’s achievements.
Critical problems affecting project completion will be brought to the attention of the
construction manager who will confer with client for recommended action.
Deviations from the specifications either will be modified to conform or resolved by
developing an acceptable solution with the proper construction discipline, subject to
client’s approval.

15. Filing Documents


The inspector shall systematically file all inspection and quality control documents for
convenient access by all SPM employees.

16. Field Supervisor’s Quality Control Activities


The inspector shall monitor the field inspector quality control activities for compliance
with the accepted procedures. The inspector shall support and guide the quality control

activities by:
(1) Obtaining the client’s ideas, evaluation, and agreement on procedures and results.
(2) Advising or being consulted by the field inspector on quality control procedures,
methods, and practices.
(3) Interpreting codes, standards, specifications, and governmental regulations.
The construction superintendent or the field supervisor will periodically conduct quality
control meetings with the craft-leader or foremen to advise them about quality control for
each category of construction work. The inspector shall always attend these meetings and
shall state procedures, programs, or other inspection and testing matters, whenever
necessary.

17. Interoffice Problem Meetings


For inspection and quality control problems, the inspector shall hold meetings with field
supervisors and other staff members to analyze the causes and results or any problems and
determine proper corrective measures.

18. Welding, Radiography, and Annealing


The inspector shall monitor and inspect welding work and all welders engaged in the field
to maintain adequate weld quality or monitor welding supervisor. He shall supervise the
progress of radiography and annealing work, coordinating the schedules with related work.
The inspector shall maintain safe operations in welding, radiography, and annealing,
especially for handling and storing radioactive materials.

19. WQT/APQ & Demonstration Tests


Work pieces or panels shall be prepared according to specification requirements. Stamps
markings or writing on work pieces must be legibly and or identifying clearly of
welder/operator names, badges, numbers, welding/application position, kinds of materials,
weather conditions, water temperatures, etc. As per each specific format to be filled in.
The inspector, client or Engineer’s representation will be pre-appointed to attend these
tests and the formats be signed by witnesses if so required after jointly inspected.
Further tests from these WQT/APQ will be RT for metal work and playsical test for
refractory work will be carried out by recognized/agreeable inspection laboratories who
will issue certificates of such tests as evidences for clients approval and references.

20. Inspection Reports


The inspector shall evaluate the results of inspections and tests and authorize any
inspection records prepared by field inspector. The inspection record and report should be
legibly written on official forms agreed upon between client and Engineer. Appendix 4 is
an inspection report form for general services to be adapted suitable for each specific job.
The inspector shall submit the inspection records to client for the inspection items
witnessed by client after reviewing by the construction manager. Other inspection records
shall be submitted to client in accordance with the agreement made between client and
Engineer.

The inspector shall periodically report the status of inspection activities to the construction
manager for every stage of inspection. At the beginning of each month, he shall also
submit the monthly field inspection report to the construction manager and the manager of
the Quality Section in Bangkok office using official forms.
2.6 SAFETY, HEALTH & ENVIRONMENT

SAFETY, HEALTH & ENVIRONMENT

1. Safety and Health

Safety and health are very importance in company’s policy in operations. Not only for its
staff, employees, and company’s properties but also aware of the others working at the same
places as well.

Accidents have to be protected by setting up precautions/regulations, and have all the


working employees to be trained prior to start working at a specific site.

The company will participate Social Welfare Program (S.W.P.) as managed by the Social
Welfare & Skill Development Ministry. Moreover, each estimator and/or project manager must
included insurance cost for buying accident insurance policy when working in
flammable/explosive production plants.

For health care, besides receiving free medical cares from the S.W.P. Each site must have a
set of first aids medicine, nominating nearby hospital/clinic, nearby for sending patient, and
contingencies for medical expenses.

The company’s target for safety man-hours is to achieve about 1,000,000 man-hours per year
without accident for the regulation workforce.

2. Environment

The company will cooperate with the Government and other parties in preserving and/or
protecting the environment, during workings and will help also by joining in making
environmental campaigns whenever available.

Materials to be used in office, works, and site, recycle products will be considered as first
priority in buying. Unused or expired materials must be kept in proper place or be sold of be
disposed.

All wastes/garbages will be disposed to proper/provided compartments or areas. Proper


protections for spillage due to wind, animals and human must be provided for container and
vehicles being transported
