AFFIDAVIT OF WAIVER

I, JESUSA CALAYAN, of legal age, Filipino citizen, and a resident of Linao

East, Tuguegarao City, Cagayan do hereby states the following:

1. That I am one of the lawful heirs of a lot located at Barrio Linao East,
Tuguegarao city, Cagayan under Lot No. 8530 Plan no. PSD- (AF)- 02-082825
containing an area of Three Thousand Four Hundred Four (3404) sq.
meters, more or less covered under CAD. 151;

Attached herein is the subdivision plan issued by the DENR

marked as Annex 1;

2. That I hereby undertake to waive all my rights and claim over the
aforementioned property in favor of the heirs, ROSALINA C. ADDURU,
SUSAN C. BATANG, NORWIN C. CALAYAN, FRANCISCO C. CALAYAN,
PACITA C. BALISI, EDUARDO D. CALAYAN, LUZ C. PINGAD, MYRNA
C.CAUILAN, NOEL CARLOS BALISI,ELEONOR BALISI PADILLA, EDDIE
CUSIPAG BALISI, EDNA BALISI AQUINO and LETICIA B. CALAYAN
including the right to transfer the tax declaration covering the said property in
their name;

3. That I am fully aware of the import and consequences of this Affidavit of

waiver;

4. Hence, I am executing this Affidavit to attest the truthfulness and the veracity
of the foregoing;

Affiant further sayeth naught.
