AFFIDAVIT

I, JOSE GALOS VALDEZ, of legal age, Filipino, married and a resident of

Luna Street, Ugac Norte Tuguegarao City, Cagayan, after having been duly sworn in
accordance with law, hereby depose and state that:

1. I am the the Complainant in the Criminal Case No 19085 entitled “People of the
Philiippines versus Tristan John Maramag et. al;
2. We, agreed on the amount to be paid for the settlement of civil aspect of the case;
3. It was agreed that the total amount of civil liability to pay was One Hundred Fifty
Thousand Pesos(P150,000.00), to be paid exactly every three months of the said
year ;
4. On May 22, 2018, I received the amount of Fifty Thousand Pesos (P50,000.00)
from Zenaida Maramag, Mary Jane Maramag and Jocelyn Maramag as partial
payment of the civil aspect of the case;
Attached herein is the acknowledgement receipt dated May 22, 2018

5. The agreed amount to be paid was P150,000.00 to be paid exactly every three
months of the said year;
6. However, Zenaida, Jocelyn and Mary Jane Maramag failed to pay the remaining
balance to me;
7. I went to their residence and give them ample time to pay me back;
8. I told them to pay me back as soon as they have the money;
9. Despite the understanding that I gave to them, they still manage to disregard my
demand;
10. That I execute this affidavit to attest to the truthfulness of the foregoing
statements and for whatever legal purpose or purpose this may serve under
the law.
