GUIDELINES FOR THE CONTENT OF ASSIGNMENT (WPA REPORT)

1. ABSTRACT
Report the identification of occupational, safety and health hazards involved in the
laboratory work in physics, chemistry and biology laboratory.
List of activities performed by the laboratory.
Risk analysis is calculated using the relationship of the severity of the accident and the
likelihood of the accident.
Proposed risk controls are introduced to prevent accidents and increase productivity and
quality of work.

3. OBJECTIVES
Identify hazards to safety and health that affect employees and students
Perform systematic risk analysis in the workplace
Recommends the appropriate safety and health measures to be taken such as elimination,
substitution, isolation, Engineering Control, Administrative Control and use personal
protector equipment (PPE).

4. METHODS
Explain at least 3 methods used in hazard identification process in laboratory
Explain risk assessment

5. RESULT AND DISCUSSION

List 6 hazard (at least 2 health hazard)
Determine the hazard based on their risk rating
Explain most critical safety and health risk identified to organization.

Explain in detail the proposed control measures in accordance to the hierarchy of risk
control (preventive and corrective) for most critical safety hazard.
