THIS IS TO CERTIFY that Sergio C. Marquina of #09 12th Street Purok 5 Brgy.
Roxas 31-D, Davao City, was examined and treated at the Southern Philippine Medical
Center on October 5, 2019 with the following diagnosis:

- The right part of the patient’s head was swollen and bleeding; and
- There were scratches in the patient’s arms

The patient needs medical attention and rest for 5 days and that would require
him to desist from performing work functions thereof.
