Important Instructions
 Applicant must be a resident of Brampton or Mississauga
 Applicant must attend in person at the above listed locations.
 Applicant must produce the one page two-sided application form and authorization letter.
 Applicant must produce two pieces of government issued identification; one which must have a
photograph and proof of address. Examples of government issued identification include:
Drivers License, Passport, Birth Certificate, Citizenship Card and Permanent Resident Card
(Health Card and Social Insurance Card, if offered). Expired identification will not be accepted.
 Record searches will be returned to you by mail. Allow 4-6 weeks to process. Processing varies
depending upon current volume.
 Record searches can also be completed “WHILE YOU WAIT” if taken to either the
Square One Community Station in Mississauga or Headquarters – Records Search Unit in
Brampton. Searches are completed on a first come, first served basis.

Information for release
 Criminal convictions from CPIC and/or local databases and Summary convictions for 5 years when
identified, where a pardon has not been granted.
 Findings of Guilt under the Youth Criminal Justice Act within the applicable disclosure period
 Outstanding entries, such as charges, warrants, judicial orders, peace bonds, probation and
prohibition orders.
 Absolute and Conditional Discharge
 Non-conviction information if it meets the Exceptional Disclosure Assessment
 Record suspensions authorized for release by the Minister of Public Safety
 Not Criminally Responsible by Reason of Mental Disorder
