Form No. 2524 * Revenue Official Receipt

TO All Internal Revenue Officials, Employees and Others Concerned

Notice is hereby given of the loss of original copy of the three (3) sets of unused
and unissued BIR Form No. 2524 - Revenue Official Receipt, bearing the following
serial numbers:

ROR201301824805
ROR201301824832
ROR201301824841

The abovementioned forms were reported as lost by Mr. Alexan C. Usngan,

Revenue Collection Officer, Revenue District Olfice No. 102. Revenue Region No. 16,
Cagayan de Oro City. Consequently, the entire ROR sets bearing the said serial
numbers are hereby cancelled. All official transactions involving the use thereof is
therefore considered as INVALID

Internal Revenue officials. employees and others concerned are requested to
