Good day!

The undersigned is writing this letter, respectfully requesting for your consent for a
replacement of participant to the SBFP Consultative meeting cum Capacity Building to the
SBFP Beneficiary Implementers S.Y 2019-2020. As that, Emelita C. Oranza, Carpenito IS
Feeding Coordinator will be replaced by Carolyn R. Caybot, Secondary Teacher I of the same
school due to the following reasons;

1. That Emelita C. Oranza will serve as one of the Board of Election Inspectors (BEI)
and,
2. That (BEI’s) are required by the COMELEC to be on- call starting May 6-10, 2019
for the Testing and Sealing of the Election Paraphernalia.
