    Dear Ma’am:

    I am writing to formally request a one (1) month leave of absence, covering the entire month of October,
    due to health reasons. I have been diagnosed with Mitral Valve Prolapse (MVP) and Gastro Esophageal
    Reflux Disease (GERD). This condition greatly affects my health and my ability to effectively do my
    daily tasks. I am currently under medication and it would be better if I can take some rest to immediately
    recover.

    I appreciate your consideration with regards to this matter. Thank you very much.

    NOTE: Proof of my medical certificate/ records will be handed personally.
