4. Telmisartan 40 mg 1 tablet once a day

5. Ketoanalogue 600 mg/tablet one tablet thrice a day

21. Epoetin alfa 4000 units SQ now then twice a week

* Epoetin alfa 4000 units SQ post-hemodialysis

23. D5050 1 vial + Actrapid 10 units for 2 cycles

* D5050 1 vial + Actrapid 10 units for 2 cycles, 2 hours apart

24. Kalimate sachet, 1 sachet thrice a day
