CONTRACTOR SAFETY SITE INDUCTION CHECKLIST

For use by Contract Supervisors (nominated UniSA staff member) to induct contractors engaged
for a defined period, ad hoc or single visit, prior to commencing work for the first time.

2. SAFETY INFORMATION FOR CONTRACTORS (mark off when complete)


Campus & Building Access:
☐ Online contractor safety induction has been completed and campus map/s provided
☐ All contractors and sub-contractors must report in and out daily at FM Assist
☐ Contractor ID card and company identification must be worn at all times while on university premises.
Security: Contact Security in the event of an emergency:
☐ using a Security Phone - several locations on all campuses are equipped with emergency wall phones for direct contact
with security staff
☐ dialling 88888 from any internal university phone OR calling 1800 500 911 (24-hours, free call)
Security staff are first aid trained to assist in a medical emergency.
Emergency Evacuation Procedures:
☐ Campus emergency plans are explained (including evacuation exit paths, locations and alarm sounds)
☐ Communicate the evacuation on alarm and to follow the instructions of the UniSA emergency personnel.
Hazard/Incident Reporting Procedures:
☐ Communicate the requirement to report any hazards that cannot be easily rectified, incidents and injuries immediately to
Security and follow up with a written report to the Contract Supervisor on corrective action taken.
Other Local Contacts:
☐ Communicate details of other key local contact personnel relevant to the planned works, including FM Assist on 55555.
Permit to Work:
☐ Communicate the UniSA Permit to Work process (WHS27) for completion and approval by Campus Facilities Manager prior
to undertaking hazardous work e.g. hot work, confined/restricted space (seek assistance from FMU or campus WHS Consultant).
Noise:
☐ Explain requirements for advanced warning of planned noisy works exceeding 50dB to be communicated to FM Assist no
later than 72hrs prior to works commencing. Such activity must take place before 8.30am or after 5:30pm.
Restricted Access Areas:
☐ Ensure restricted access areas are identified and notified to FM Assist. Approval must be sought from the Campus Facility
Manager and Manager of the specific work area (i.e. laboratory/workshop/studio).
Electrical Equipment:
☐ Communicate the requirement for any power tools, extension leads and associated portable electrical equipment to
have a current test tag attached prior to use on UniSA premises.
Hazards:
☐ Communicate any known hazards associated with the work location and surrounding areas and the method of control.
General Welfare & Facilities:
☐ Communicate the location of the closest amenities i.e. toilets, wash facilities, dining facilities.
Vehicles:
☐ Inform of the parking requirements and speed limits in and around the campus.
3. SIGN OFF (Provide a copy of completed checklist to the contractor and retain original with records for this service contract.
