STANDARD OPERATING PROCEDURE

Radiology Department

 Receive, assist and mobilize patients in accordance with the physical and mental
condition of each patient appropriately for radiographic procedure;
 Secure in-patient or out-patient request for or given by the attending physician;
 Double check patient’s data:
- Full name
- Age, date of birth
- Home address
- Contact number
- Patient’s history (Chief Complain)
 Specify and clarify which radiographic procedure needs to be done;
 Inform and instruct patient regarding the manner on which the procedure is to
be done, as a preparation and to serve as verbal consent;
 Prepare all equipment needed as well as the patient for the procedure:
Ex. for Chest X-ray
1. Prepare cassette and film needed.
2. Instruct patient to remove upper clothes, any metallic materials and
put on the appropriate laboratory/patient’s gown.
3. Position patient, equipment and associated devices.
4. Manipulate technical factors to achieve high quality images for
interpretation by producing image parameters (MA, KV, Time,
Distance, etc., or any other combinations thereof).
 Apply principles of radiation protection to minimize radiation exposure to
patient, self and others;
 Routinely inspect on regular basis, all equipment under control and make minor
adjustments as deemed necessary;
 Stock and assure availability of supplies in conjunction with the radiologist,
administrator/management or pharmacist;
 In case of equipment malfunction, report immediately to supervisor or
management;
 In releasing the Official Result, the following must be observed:
- Always secure a triplicate copy for departmental filing;
- Double check and verify the right results for the right patient;
- For Medico-legal case patients, attending physician must be informed
before releasing the original copy of result only;
- For in-patient, Duplicate copy of Results must be attached to the
patient’s chart. Original copy is released together with the x-ray film as
soon as result is available;
- Original and duplicate copy of results, together with the x-ray film is
given to the patient/claimant. Claimant/patient must sign on the
patient’s (receiving) log book on the space provided for; and,
 DO THE RIGHT THING RIGHT THE FIRST TIME!
