Proof load test:

 A test weight of 500kg was used and the device made to suspend for 20minutes. Test was
successful; there were no changes or deformation in the holding devices and the platform as a
whole.

Deficiencies:
 nil

Recommendations:
 Anti-collision device should be fixed at platform sideways.
 Heavy hammer is a weight attached to the lower end of the safety wire rope, this is to give it a
weight for easy movement through the safety lock device, will need to be provided.

Conclusion
The equipment inspected is in good working condition and conforms to the standards above; I therefore
certified the equipment for six months use.
