This is to request permission for a round trip airfare from Tuguegarao City to Legaspi City on
October 6 and October 11, 2019 in our attendance to the Consultative Conference of CLMD
Chiefs and Regional Supervisors in-Charge in Campus Journalism to cope up on time with the
scheduled activities before and after our travel. Furthermore, we need to utilize the downloaded
fund amounting to P25,000.00 to cover transportation allowance of two (2) pax.
