PETITION
Petitioner, through the Public Attorney’s Office, Bais City District, unto this Honorable
Court most respectfully states that:
Respondent Catherine Chong is brought as formal party in this petition in her official
capacity as the City/OIC Civil Registrar of Manjuyod, Negros Oriental, Philippines with office
address at City Hall Annex, Bais City, Negros Oriental, where she may be served with summons
and other court processes;
That petitioner was born on September 8, 1968 at Cambuilao, Bais City, Negros Oriental
to parents Margarita Bocado and Primo Nipaya;
The fact of petitioner’s birth was duly reported and recorded at the Civil Registrar of Bais
City, Negros Oriental, the Certificate of Live Birth is hereto attached and marked as Annex “A”;
That she grew up believing her parents named her Nina Maria, as per her baptismal
certificate, which was in their possession. That because of this, all her government transactions,
including her getting married, her name is Nina Maria;
That she was astonished to find out that she was named Nena in her birth certificate;
That because of this discrepancy, there is a huge confusion as to her name, as she has
been using the name Nina Maria since she was a child, and she grew up believing that her name
is really Nina Maria;
That to support her claim that she has been using the name Nina Maria, attached is her
Baptismal Certificate, Voter Certification, Police Clearance Certificate, NBI Clearance,
Certificate of Marriage, Birth Certificate of Mary Joy Ojeda and Birth Certificate of Maria
Anthonet Ojeda, herein attached and marked as Annex “B”, “C”, “D”, “E”, “F”, “G” and “H”
respectively;
Unless and until such error is corrected, petitioner will have a hard time adjusting her
status due to the confusion, thus cannot attain a complete convenience in her life and livelihood;
Petitioner has no pending case or cases in court for violation of any law or any civil
liability and that the petition is not filed in avoidance of any criminal or civil liability;
WHEREFORE, in view of the foregoing, it is most respectfully prayed that after hearing,
judgment shall issue as follows, to wit:
1. Ordering the change of first name in the birth certificate of the petitioner referring to
her first name from Nena to Nina Maria;

petition. That I caused the preparation of the same and know the allegations therein
to be true and correct to my knowledge and belief.

On oath, as petitioner, I hereby certify that I have not commenced any action or
proceeding involving the same issue in the Supreme Court, the Court of Appeals, or
any tribunal or agency. To the best of my knowledge, there is no such action or
proceeding pending before such above-mentioned agency. Should I learn that a
similar action or proceeding has been filed or still pending before such agencies, I
undertake to report the same to this Honorable Court within five days from
knowledge thereof.
