Purpose: Exhibits “A” to “B” and their sub-markings are


being offered in evidence to prove compliance
with jurisdictional requirements and to prove that
the Honorable Court has jurisdiction to hear and
decide the present petition.
C Condominium Certificate of Title No. C-5404
C-1 Memorandum of Encumbrance
Purpose: To prove that the Petitioner is the registered
owner of the subject property.

To prove that Petitioner has made the necessary


annotation of the subject property with the
Register of Deeds of Valenzuela City with regards
to the loss of her owner’s duplicate copy of the
condominium certificate of title.

D Condominium Certificate of Title No. C-5405


D-1 Memorandum of Encumbrance
Purpose: To prove that the Petitioner is the registered
owner of the subject property.

To prove that Petitioner has made the necessary


annotation of the subject property with the
Register of Deeds of Valenzuela City with regards
to the loss of her owner’s duplicate copy of the
condominium certificate of title.

E Affidavit of Loss for Condominium Certificate of


Title No. C-5404
E-1 Affidavit of Loss for Condominium Certificate of
Title No. C-5405
Purpose: To prove that the Petitioner has lost her owner’s
duplicate copy of the subject condominium
certificate of titles.

F Certification from the Land Registration Authority

Purpose: To prove that per registry records of the Land


Registration Authority that Condominium
Certificate of Title No. C-5404 and Condominium
Certificate of Title No. C-5405 under the name of
the Petitioner are still existing and intact.

G Tax Declaration No. C-031-21352


Purpose: To prove that Petitioner has declared
Condominium Certificate of Title No. C-5405 under
her name for real property tax purpose.

H Tax Declaration No. C-031-21353


Purpose: To prove that Petitioner has declared
Condominium Certificate of Title No. C-5404 under
her name for real property tax purpose.
I Clearance of Real Property Tax
Purpose: To prove that Petitioner has paid all due real
property taxes with regards to the subject
properties

J Certified electronic true copy of Condominium


Certificate of Title No. C-5404
J-1 Memorandum of Encumbrance
Purpose: To prove that the machine copies presented by the
Petitioner are identical to the copy in the
possession of the Registry of Deeds of Valenzuela

K Certified electronic true copy of Condominium


Certificate of Title No. C-5404
K-1 Memorandum of Encumbrance
Purpose: To prove that the machine copies presented by the
Petitioner are identical to the copy in the
possession of the Registry of Deeds of Valenzuela

L Affidavit of Loss
Purpose: To prove that the Affidavit of Loss presented by
the Petitioner is identical to the Affidavit of Loss in
the records of the Registry of Deeds of Valenzuela.

All the above enumerated exhibits are already on file with the
record of the above case.

WHEREFORE, it is most respectfully prayed of this Honorable


Court that the foregoing pieces of evidence be admitted for the purposes
they are being offered.

With the admission of the foregoing pieces of evidence and the


oral testimony given by the witnesses of the Petitioner, the Petitioner
rests her case.
