Petitioner, thru counsel, unto this Honorable Court, respectfully

1. On May 21, 2019, Petitioner filed a Petition for the CORRECTION

OF REPORT OF MARRIAGE WITH REGISTRY NO. 2011-
3922051;

2. Petitioner respectfully manifest that a copy of the petition was

furnished to the Office of the Solicitor General and the Office of
the City Prosecutor of Quezon City thru LBC Courier Service by
the undersigned counsel in view of the time constraints and
shortage of available manpower to effect personal filing and
service;

3. As proof of the above services, a copy of the official receipts issued

by LBC Courier Service is herein as “Annex A” and “Annex B;”
