AFFIDAVIT OF UNDERTAKING

I, JOSIE ANN ASTILLERO y POGAY, 25 years old, Team Leader at

Jollibee Green Medows and residing at Block 2 Fork St., West Floodway, San
after being duly sworn in accordance with the law do hereby depose and states;

1- That, I am the sister of JONJIE ASTILLERO y POGAY, 28 years old,

married, welder and residing at Blk. 4, Barangay San Andres, Cainta, Rizal who
is presently confined at Quezon City General Hospital, Quezon City due to
multiple stabbed he sustained on August 26, 2018;

2- That, Jonjie P Astillero was the owner of one (1) Honda TMX 1559
motorcycle with engine number KB509E044664, chassis number KB509044663
bearing plate number 5575PT;

3- That, the said cited motorcycle was been mentioned and involved in the
stabbing incident that transpired on August 26, 2018 at about 8:30 PM, inside
Dos Bente Uno KTV Bar located along C-3 Rd., corner A. Mabini St., Caloocan
City;

4- That, in this connection, I am requesting for the release of the afore-

described motor vehicle and bind myself FULL and complete responsibility to
present the same whenever required by the court and/or any investigation
agency anytime;

5- That, I am executing this affidavit and I have hereunto affixed my

signature to attest to the truth to veracity of all the foregoing.
