Dear Sir/Madame:

We are writing to you in connection with our intention to claim the Death Benefits to
which we are entitled as primary beneficiaries of our father, MR. DOMINGO O. ANG, who
passed away in April 4, 1994, and who is a Social Security Member with SSS No. 03-0580241-1.

Our mother Mrs. Bee Tek Ang, who is Mr. Domingo’s wife, has also already passed away
in August 20, 2016. As such, the four (4) of us as siblings are the only remaining heirs of our father
Mr. Domingo, and are the ones entitled to claim his death benefits under the Social Security
System. Unfortunately, ever since the death of both our parents, we have not had any
communications with our sibling MR. DAVID ANG. As such, in order to not anymore delay our
right to claim our father’s death benefits, the three of us siblings have decided to claim our
respective shares in the said death benefits ahead of our brother Mr. David, who may
subsequently decide according to his own preference as to when he will claim his own.

With this, we would like to ask for your good office to kindly allow instead the three (3)
of us siblings to claim our respective shares in our father Mr. Domingo’s death benefits to which
we are entitled, namely:
