Dear Sir/Madam,

I'm Mr.Vincent Beloria Edem whose currently 20 years Pag-Ibig Fund Member's since year
2000.Sir/Madam,Please accept and process my withdrawall request all of my Pag-Ibig Fund Membership
Contribution please help me.

I've been working as a private employee Sir/Madam for almost 20 years(Two Decade),
After a though and discussion with my family I feel the decission to never back to working again as a
private employee and never find again for another job 20 years as a private employee it's enough for me.

It's my final decision to leave here and,It's time to go back to my Home Province,Agdao,Davao City and
I'll stay here for good.

Before,I'm back to my Home Province,Davao City please Sir/Madam,I need your help and support my
request of withdrawall all of my Pag-Ibig Funds Membership Contribution.

I really need some financial assistance sir/madam right away because my plan,I'm start my own small
business in Agdao,Davao City to help my family because my father we have a cancer sa atay po
sir/madam kaya isa rin po yan sa mga dahilan kaya nag decide ako to go back na sa davao po para
matulungan ko mama ko sa pagbabantay sa papa ko po kasi may taning na buhay niya kahit papaano nasa
tabi niya po ako.
I need your support sir/madam please help po regalo nyo na po sa akin sana this coming christmas po para
sa Agdao,Davao City na po ako magpapasko makasama ko na mama at papa ko for the first time po
sir/madam.

But this time in my life my decision to withdraw my contribution it's hard for me but we have no choice
it's my family decision dahil para sa akin family first bago ang sarili..
