Rules and Regulations of the College of Nursing and Health Sciences

I. ADMISSION
All the students of the College of Nursing are enjoined to comply with the rules and regulations

In order that a student may be considered for enrolment in the Department, the applicant has to meet
the minimum requirements of:

1. The average of Highs School grades and/or academic grades of the college applicant must not
less than 80% with a grade of 80% in the following subjects: Math, Science English.
2. Satisfactory results of the psychological test given by the guidance center.
3. Birth Certificate – original copy.
4. Certificate of good moral character issued by the parish priest, principal or guidance counselor
from the school last attended.
5. Pleasing personality, no physical defect and with general liking with people particularly with the
poor and the sick.
6. Passed the personal interview
7. Physically fit. (Clear X-ray result and NPE)
8. Obtained a passing score in the NAT (Nursing Aptitude Test) which is conducted at the second
semester of the year. If the student did not pass the NAT, he/she has to obtain a grade of @
least 83% in all major nursing subjects.

II. ENROLMENT PROCEDURE

OLD STUDENTS:
1. Evaluation of Grades
2. Medical Examination with X-Ray, stool and urine exam
3. Secure enrolment form.
4. Refer to schedules of subjects at the bulletin Board
5. Confer with dean of college/ VP for academics
6. Proceed with enrolment procedure.

NOTE:

Official enrolment is attained only when class cards are given to the particular teacher-in-charge
of the subject to be taken.

III. READMISSION

Those who discontinued the course due to health problems, pregnancy may be re-admitted after
submission of a Chest X-ray and medical certificate after giving birth issued by the school physician that
she is found physically and mentally fit and capable of pursuing the BSN course. This is taken on a case
to case basis.

IV. DISCONTINUANCE OF THE COURSE


A student who wishes to discontinue the BSN course shall write the Department Dean stating whatever
the reason is for discontinuance

V. GROUNDS FOR STUDENT DISMISSAL


A student is dismissed on the following grounds even on the basis of a first attempt after due
deliberation.

IMMORTALITY – This includes illicit relationship with male/female before and during marriage including
intentional abortion, STD’s

The following situations are deliberated upon a case to case basis. A first offense may serve as warning
depending on the gravity of the offense and the malice included in the commission of the act.

1. Dishonesty/Cheating – When a student is caught cheating with all the evidences, student gets a
failure in that subject where students cheated
2. Defiance or disrespect of person in authority
3. Absence of 30% of the subject hours, 3 accumulated absences excuse or unexcused, parent or
guardian will be called and 5 accumulated unexcused absences in the RLE. This will be depend
on the subject and the RLE the student was absent form.
4. Pushing and using/carrying of prohibited drugs and other offense such as:
a. riot, involvement in gangs, fraternity/sorority , cult
b. Carrying deadly weapons
c. Drunkenness
d. Vandalism
e. Hooliganism/prank
f. Threatening or assaulting a person in authority, teacher or fellow student
g. Instigation leading to interruption or stoppage of classes
h. Forgery or tampering of school records.
i. Damage of campus/school facilities
j. Damage of the name and the department thru print and broadcast media.
5. Other grounds are specified in the guidelines of the General college as printed in the student
handbook.

VI. DECORUM AND BEHAVIOR

A. In the School

1. Wear the required uniform with valid ID everyday and on prescribed occasion
2. Maintain silence within school premises and corridors especially near the chapel
3. Take good care of facilities, furniture and apparatuses and keep them clean and functioning
at all times. Damages, losses and leakages are paid. Report immediately damages, losses
and leakages. The whole class will have to pay if not reported. No clearance will be signed
unless all of the above reckoned with.
4. Smoking inside the campus and RLE areas 1 km within school vicinity is never allowed and at
all times.
5. Visitors are allowed only in extreme emergency cases when schedule and activities permit.
This is allowed only upon expressed permission from the Dean of Nursing. At other time no
visitors is allowed in the school or in the school area.
6. Attend classes and other school activities on time and at all times.
7. Avoid loitering around the school, community and hospital during class/duty.
8. When absent/tardy, present excuse letter to the department head with a written
explanation of such from your parent or guardian. Present excuse letter signed by office of
student affairs teacher whose classes the students have missed. No excuse letters means a
student will not be allowed to attend the class.
9. Pay necessary dues and accounts on time.
10. For any problem personal or otherwise, consult the Year Level Coordinator or Guidance
Counselor.
11. Consult school physician when one becomes ill in school or the School Nurse if Physician is not on
duty. (Notify the school clinic during the time you get sick, for record purposes and for the validation of
your medical certificate upon going back for classes and/or duties.)
12. Return or report immediately to the Nursing Office anything found or lost.
13. Keep the campus and classrooms clean and presentable at all times.
14. Respect, honesty and sincerity are virtues proper of future nurses and are most expected of students
at all times.
15. Cell phones are strictly prohibited in the classroom as it will distract classes. Found out using cell
phones in the classroom means confiscation. First Offense will be for one week, second offense for one
month and third offense will be confiscated, returned to the student at the end of the semester.

B. Outside the School

1. A Trinitian upholds the mission statement in his/her speech manner and conduct in any situation
whatever student should project an image of a real educated and refined students at all times.
2. AHSE and Nursing students should refrain from wearing the School Uniform in any places like movie
houses or to other indecent places not deemed respectable by the school.
3. Appearing on TV shows and advertisements are strictly prohibited and discouraged, unless permitted
by school authorities.

C. In the Clinical Area/Community

1. Nursing Student should report promptly in complete uniform 15 minutes before scheduled time.
Reporting to duty after 15 minutes is considered absent from duty and with a sanction of 1:3 with
consideration to skill lab.
2. In the clinical area, one should completely carry out assigned activities with the supervision of the
Clinical Instructor and form a good habit of seeing Christ in the patients. Related Learning Experience is
not only a means of learning but an opportunity to actually put into practice the mission statement of
the school.
3. Professional decorum is highly encouraged and expected at all times. Students must treat/address
each other professionally.
4. Loud talking, laughing, giggling, etc. should be avoided within the community/hospital premises or
whenever one is on duty.
5. Visits/telephone calls are discouraged in all RLE areas where students are assigned.
6. Cell phones, Laptop and other gadgets are strictly prohibited in all RLE areas as it will distract work.
Found out using elec gadgets means confiscation. First offense will be confiscation for one week, second
offense for one month confiscation and third offense will be confiscated and to be returned to the
student at the end of the sem.
7. if one suddenly becomes sick while on duty, notify the clinical instructor for possible immediate
referrals to ROD. If confinement is needed, the family is notified and the student chooses where he/she
prefers to be admitted.
8. Nursing Student render total health nursing care and implement the nursing process which is always
countersigned by the Clinical Instructor and head Nurse of the unit.
9. Students who report for duty to another ward/or clinical instructor instead of the assigned area shall
be marked absent.

VII. School Uniform

1. Female Uniform
A. White full dress according to cut and design with valid ID.
B. All underwear should be white or skin tone (chemise, brassiere, panty)
C. Any black close shoes at least one and a half inches to two inches tall heels should be worn.
Wedge and boot type shoes are not allowed.
D. Skin tone stocking should be worn.
E. BLACK BAG should be used in the school and RLE areas.
F. Hair should be fixed in a bun using only black accessories
G. Only colorless nail polish is allowed
H. Only functional wrist watch with second hand pearl stud earrings are allowed

2. Male uniform
A. Prescribed white uniform with name plate attached to the shirt pocket and a valid ID.
B. Plain white under shirts should only be worn under the uniform and should be tucked in.
C. Black formal shoes with white socks for daily classes.
D. BLACK BAG should be used in the school and RLE areas.
E. Skin head, highlights, hair color is not allowed.

VIII. RLE Uniform


A. Hospital, Ward, ER Uniform

It is reasonably assumed that all students enrolled at Holy Trinity University did so, out of their own
will have therefore assumed obligation to abide by its requirements. This uniform should follow the
official cut and design by the school and is not subject to the fads and fallacies of the fashion. Students
must come to school or report for duty with the required complete uniform wherever they may be
assigned. A complete uniform includes:
a. Clean white uniform and apron and nurse cap in the ward for female nursing students and
clean white uniform with nurse pin.
b. Clutch bag for V/S paraphernalia’s and TL should bring PHN bag for the group.
c. PHN Bag with the following
1. Bandage scissors
2. Pencil box with sets of ball pen, eraser, small ruler, pentel pen.
3. B.P. apparatus and stethoscope
4. Oral, axillary and rectal thermometer
5. Nail cutter
6. Tourniquet
7. Syringes and needles
8. Gloves
9. Bonnet and mask
10. Receptacles with dry cotton balls with alcohol, cotton balls with betadine and cotton
balls with hydrogen peroxide
11. Kidney basin
12. 2 kelly forceps, surgical scissors, needle holder, thumb and tissue forceps
13. Gauze and plaster
14. Tongue depressor
15. Medicine glass
16. 6 hand towel, 2 face towel
17. Medicine glass
18. Soap and soap dish
19. Wrist watches with second hand
NOTE: all thermometer should be digital

B. OR, DR Uniform
1. Prescribes scrub suit with nameplate attached to the left side of the breast. Smock gown
should be worn in the sterile areas.
2. Prescribed bonnet and mask
3. Student should report in the area (OR, DR) in their hospital uniform with apron
4. Students are required to bring clean prescribed slippers

C. SKILLS LAB UNIFORM

1. Prescribed scrub suit with valid name plate, white duty shoes and socks.
2. Long hair should be fix in a bun, and short hair should not touch the collar.
3. Should bring the nursing procedure manual.

D. COMMUNITY UNIFORM

1. Prescribed P.E shirt and denim pants with valid name plate.
2. Black rubber shoes
3. If jacket is worn only plain neutral color (black, white, brown and gray) is allowed.
4. Only black, non-folded umbrella.
5. Complet6e set of PHN bag.
6. Bring Nursing manual as needed.
7. Hair should be fixed in a bun.

IX. PERSONAL GROOMING

1. Hair enhances the beauty of the face. If worn short, it should not touch the eyebrow and collar. If
long hair is preferred, it should be neatly bunned or brushed up. Only BLACK hair ties are allowed.
COLORED hair ties should be confiscated.
2. Only light lipstick with light make up for females are allowed.
3. Beard/mustache are not allowed.
4. Nails are well trimmed and clean. Natural nail polish may be the only kind allowed.
5. Cleanliness and freshness of the entire body and freedom from body odor are marks of an attractive
person.
6. Stocking must always be clean and run free. Shoes must always be spotless clean.

X. ATTENDANCE AND PUNCTUALITY

Each student should accomplish the required total number of hours on the subject enrolled as well as
the total number of hours required in the Related Learning Experiences as indicated in the curriculum.
Every student nurse should develop the virtue of punctuality. Arriving 15 minutes after the bell has rung
is considered late.
Rooms will be closed. After 15 minutes and you will be considered as absent. If a student suddenly feels
ill and get absent, the student is required to notify the class adviser by written note, or by school phone
or by cell phone. If the student has been sick, a medical certificate signed by the school physician and a
letter coming from the parent or guardian should be presented. A tardy excuse slip from the student
affaires secured. Without these the student may not be admitted back in the classroom. Five (5)
unexcused absence shall mean a final grade of 70% in the particular subject where the student has been
absent from.
In cases of 15 minutes tardy in R5LE areas are considered absent and should be duty sanctioned, 3
accumulated tardiness in the clinical area must be sanctioned as unexcused absence.

XI. ABSENCES

An absences with reasonable cause should not exceed 6 duty days or classroom days. 3 absences,
parents will be called or notified.
Justification of reasonable absences are as follows:

1. Sickness of the student himself/herself.(except dysmenorrheal, LBM, toothache, headache)


2. Serious illness or death of either the mother, father, brother or sister.
3. School activities representation with permission from the Dean of Nursing.
4. Other causes shall be dealt with on a case to case basis, with the Dean.

The Instructor should check the student daily before the start of the class. In the Clinical Area, the team
leader coordinates with the CI regarding with the attendance of the group members. A student must
present an letter of excuse signed by the parent/guardian and secure an excuse slip from the level
adviser before resuming his/her duty.
For any unexcused absences in the clinical area, the student pays 3-5 days of consecutive duty
determined by the Clinical Instructor and College Dean. For unexcused absences in the Clinical Area, the
student pays one day of duty. Three late is equal to one unexcused absence.

Types of Sanctions
Excused Absence - 1:1
Unexcused Absence – 1:3
Unexcused Absence during special/major activities – 1:5 such as trainings, grand case, culmination
3 accumulated tardiness = 1 unexcused absence

XII. IN THE CLASSROOM

1. Classes are started with an opening prayer and end with a closing prayer. Bible reading is also
recommended.
2. Attendance is checked.
3. Activities in the classroom are dependent upon the creativity of the instructor and the readiness of
the student to participate actively.
4. Skills laboratory are scheduled separately in groups by then it is equivalent to a class hour.
XIII. TARDINESS

Arriving 15 minutes the class was started, a student considered late. If a quiz is given continues and
arrives late, she is allowed to take the quiz on the number or item she is able to catch up with. Test
questions are not repeated for the student.

In RLE, a student is considered late the attendance is checked before duty. If a student continue to
be tardy despite warnings and calling of attention, then a more disciplinary action taken. At this
point, the parents may be called for a conference with the perfect of discipline and the Dean of
nursing.

DISCIPLINARY PROCEDURE

The following are considered violations of the school rules and regulations of the department.
The students are made to understand why violations of these rules and regulations shall subject the
offender to such corrective measures as may be imposed by the authorities of the department after
taking into considerations the circumstances of each particular offense.

A. 1. Irregularities

a) Habitual tardiness
b) Cutting classes/ duties
c) Skipping from department’s school activities

2. Improper use of uniform


3. Unruly conduct in the classroom/clinical area premises, library, IRC, Corridors, Computer
Room, Canteen,
4. Littering
5. Collection of money without Nursing Office permission.
6. Prolonged snack time.

B. 1. Any student dismissed because of attitudinal problems or gross Misbehavior


a) Is not given any recommendation or certification of previous attendance in school.
b) Is not admitted back in the same college at any time unless matters have been dealt
thoroughly with the discipline committee.

XIV. CANDIDATES FOR GRADUATION

Application for graduation should be filed not later than October for the first semester and February for
the second semester and immediately after the close of enrollment during summer. The graduating
student should have at least 80% of credit in the major nursing subject and the thesis abstract in the
nursing field.

Academic
1. Completion of the units required for the course.
2. Clinical/RLE
2.1 Completion of number of RLE hours in all level.
2.2 Completion of OR and DR cases.
3. Submission of the thesis abstract/proposal in any major nursing field.

XV. IN THE CLASSROOM


1. Classes are started with an opening prayer and end with a closing prayer Bible reading is also
recommended.
2. Attendance is checked.
3. Activities in the classroom are dependent upon on the creativity of the instructor and the
readiness of the student to participate actively.
4. Students are required to secure textbook recommended by the department and to be brought
in the classroom. No textbooks may mean no grade in the group work or recitation or in the
activity given by the instructor.
5. Skills laboratory are scheduled separately in groups by then it is equivalent to a class hour.
XVI. TEXT AND EXAMINATION
Quizzes and long test and practical exams are given regularly before the end of each term, the prelims
midterms and finals. Failure to the above examination and/or failure to complete course requirements
such as term papers, reports, case studies, etc. part of the subject justices a grade of failure is a final
grade.
EVALUATION OF STUDENTS/GRADING SYSTEM FOR MAJOR NURSING SUBJECTS
Students are regularly evaluated both in the classroom and RLE to ensure that competency based
conceptual and developmental approaches and learning are adequately achieve. Classroom evaluation;
please see method of grade computation).
Related Learning Experience Evaluation; the student are closely supervise by the CI.
The bases for evaluation are:
A. Performance in valid clinical activities. . . . 50%
B. Ind/grp. Nursing care analysis, written
assignments ward classes and quizzes. . . . 20%
C. Completion of clinical requirements. . . . . . 10%
D. Attitudes and behavior. . . . . . . . . . . . . . . . . 20%
a. Punctuality
b. Grooming
c. Respect

Performance evaluation per concept are filed in an individual in folders every after and RLE is
finished, duly graded and signed by the student and the CI. For academic grades, the
department follows the computation of grades of the entire college.
(please refer to college student handbook).

Clinical/related learning experience is given a separate grade covering all the areas of exposure
of the student.

In the final computation of the academic grades, 50% is given to the total clinical/RLEskills lab
grades and 50% of the classroom grade.

Failure in theory will automatically gain a failing grade in RLER.

This explain why major nursing subjects the student have two grades. One grade for the
classroom and another grade for the clinical/ RLES kills/ laboratory.

The final passing grade in academic is 80% the passing grade in the Clinical/RLE/Skills is 80% in
one of the clinical area, you will repeat exposure in the area where you failed.

In case the student fails in the major subjects, student maybe allowed to repeat the subject the
next following semester until she has passed. This will explain why she may be delayed for
semester tom 1 year and she will not be able to join the original group. The student will only
allow to repeat the subject once. If again she/he failed she/he will be advised to discontinue the
course.

XV11. AWARDS

1. Florence Nightingale Award Excellence in clinical performance in all area with minimum
average of 91%, no below 85% (for graduating)
2. Excellence in Community Health Nursing Excellence in the use of nursing process in
community health experience-outstanding in community involvement student has to
accomplished relevant community projects, implementation and evaluation of plan
during the entire community exposure.
3. Special departmental award in the clinical/RLE – a grade of 91% with no grade below
85%. Excellence in RLE – student highest grade earned from first semester of AHSE II to
second semester of BSN IV in the clinical area.

XVIII. ACTIVITIES

A. Professional Activities. Philippine student association professional leadership qualities,


awareness of need for citizenship participation and to train students in parliamentary
procedure and administration organization.
B. Holy Trinity University Nursing Student Association will be automatically a member body of
student in the department of nursing

B. Religious activities
Student catholic nurses association
Standing committee
Pray together committee
Play together committee
Study together committee
Work together committee
All students are obliged to attend all activities of the department and the school.
