AFFIDAVIT
FOR CHANGE OF GRADE

I, Agguid, Myrna Joy Mariano, of legal age, single, Filipino and a

resident of Magsaysay, Bayombong, Nueva Vizcaya after having
been sworn to in accordance with law hereby depose and say:
1. That I am a student of Nueva Vizcaya State University;
2. That I was enrolled in the subject Consumer Behavior on the
1st semester of the schoolyear 2016-2017;
3. That I failed to complete the subject because I failed to pay
my remaining school balance of P8,000.00 before due to the
lack of funds;
4. That I was given the remarks given to me was NTP (No Test
Permit) and thus considered incomplete;
5. That I am executing this Affidavit freely and voluntarily without
any reservation to attest to the truth of the foregoing narration
of facts for whatever legal intent and purposes that it may well
serve.

IN WITNESS WHEREOF, I have hereunto set my hand this 14th day

of January, 2020 at Bayombong, Nueva Vizcaya.
