PROMISSORY NOTE

KNOW ALL MEN BY THESE PRESENTS:

This PROMISSORY NOTE is executed by ___________________________, who

is of legal age, Filipino citizen and a resident of ________________________________,
hereinafter referred to as the PROMISOR, in favor of __________________, who is
likewise of legal age, Filipino citizen and a resident of ______________________________,
hereinafter known as the CREDITOR, and both of whom by virtue of this instrument,

WITNESSETH
WHEREAS, this instrument is executed by the PROMISOR in favor of the CREDITOR for
and in consideration of a LOAN granted by the latter in favor of the former, in the amount of

currency delivered by the CREDITOR and received by the PROMISOR at

WHEREAS, the PROMISOR agrees to pay an interest of ______________________
