PROMISSORY NOTE

FOR THE VALUE RECEIVED, I/WE, the undersigned makers and co-makers, jointly and severally, promise to pay to the order of
HOLY CHILD MULTI-PURPOSE COOPERATIVE (Holy Child MPC), A cooperative registered under the laws of the Republic of the
Philippines with principal address in 620 T. Claudio St., Bato, Leyte, the sum of
(P ), Philippine currency, with interest at the rate of % for computed
from the date hereof until full payment thereof starting on until , in
installments based on the Amortization Schedule, hereto attached as annex “A” and shall form as integral part hereof.

In case of any default in the agreed payment schedule and/or any deviation of the loan proceeds, the payee or its assignee,
endorse is unconditionally entitled to declare all unpaid balance of the note immediately due and payable. Notice of dishonor is
expressly waived. A penalty charge of Three percent 3% per month shall be charged on all delayed or unpaid installments & Five
percent 5% per month of unpaid balance for overdue loan. The cooperative is authorized to set off or apply any deposits in the
cooperative in borrower’s name, account to the payment of the loan without need for prior notice or approval from the member-
borrower and/or co-maker;
In case this is referred to an attorney for collection or legal action, I/We bind ourselves to pay attorney’s fees equivalent to ten
(10%) of our outstanding obligation to any holder hereof, exclusive of costs and expenses of litigation; but, in no case, less than five
thousand pesos (P 5,000.00). Any action on this instrument shall be brought before proper court in MCTC, BATO, LEYTE Philippines.
