PHOENIX CORPORATION, a corporation duly organized and existing under the laws of
the Philippines, with office address at Unit C26 Third Floor, 9th Avenue Corner, 36th Street,

I am hereby requesting for the cessation of my business registration in your barangay because
the business is no longer existing.
