Respectfully transmitted herewith is the request for the release of Notice of Cash Allocation in the
amount of One Million Six Hundred Two Thousand Pesos (1,602,000) of PENRO Davao del Sur
to cover payments of due and demandable accounts payable under Fund 401 with MDS Trust
Account No.2033-9012-60.

Please see attached Monthly Disbursements Program for FY 2019 and List of Due and Demandable
Accounts Payable – External Creditors, duly signed by the authorized officials to support their
request.
