CONTRACT OF LEASE

KNOW ALL MEN BY THESE PRESENTS:

I, LESLIE B. DY, of legal age, Filipino, married, with residence and post office address in
No. 258 Zone 5 Tagas, Tabaco City, hereby LEASES unto Ms. MARICEL FERNANDEZ, single,
and likewise of legal age, that one (1) unit residential house located in Block 21, Lot 13, Moran
Estate Subdivision, Matagbac, Tabaco City and covered by TCT No. T-108125, under the following
terms and conditions, to wit:
1. That the parties hereto have entered into a Lease Contract covering the use of
aforesaid one (1) unit residential house at a monthly rental of Seven Thousand Pesos
only (Php 7,000.00) for a period of one (1) year from October 29 , 2019 to October
29, 2020 and renewable at the will of both parties;

2. That the LESSEE shall give to the LESSOR at the time of the signing of this CONTRACT
an amount equivalent to one (1) month deposit and one (1) month advance payments,
which amount shall not be consumed as monthly rental but shall be reserved to answer
for unpaid arrears in the payment of utility charges which the lessee is accountable
for;

3. That the succeeding monthly rental shall be paid in advance every 30th day of the
succeeding month;

4. That the LESSEE may, with the previous written consent of the LESSOR, introduce at
the former’s expense, improvement/s on the premises leased, provided that at the
termination of this LEASE CONTRACT, the said improvement/s shall become the
property of the LESSOR and shall remain on the premises without reimbursement
therefor; and provided further, that the said improvement/s shall not alter the form
and structure of the leased premises;

5. That all utility charges such as electricity, telephone, water, etc., shall be account for
the lessee;

6. That the LESSEE shall return to the LESSOR the property leased therein immediately
upon the expiration of the period termination of the lease contract;

7. That during the term of this Contract, the LESSEE shall comply with the laws and
ordinances relating to the use, care and maintenance of the leased premises;

8. That the LESSEE shall not assign nor sub-lease the premises without the consent of
the LESSOR;

9. That the LESSEE shall keep the leased property in good and habitable condition and
shall be responsible for the necessary repairs thereon when a need arises at his/her
expense.

10. That the LESSEE shall keep the property herein clean and sanitary condition;

11. That any violation of any of the terms or conditions stated herein shall cause the
termination of this contract, without prejudice for the aggrieved party to demand the
consequent damages therefor;

12. That the LESSEE shall notify the LESSOR in writing within 30 days prior to the
expiration of this contract of his/her/their intention to renew this Contract.

IN WITNESS WHEREOF, we have hereunto set our hands this __th day of ____________,

LEASE consisting of two pages including this page, signed by them and their witnesses on each
and every page thereof, and acknowledged to me that the same is their free act and voluntary
deed.
