REQUEST FOR BUDGET

Dear Mrs.President,

The Mathematics Department will participate in the Division

METROBANK MTAP ELIMINATION ROUND on January 17, 2020 at Lanao Norte
National Comprehensive High School in Baroy, Lanao del Norte. In view of this
activity, we are requesting for a financial assistance from the PTA fund for the fare,
snacks, and meals.

We are hoping for your favorable action regarding this request.

Here is the proposed budget for the fare, snacks and meals.

FARE ---------------------------------------------- 50 X16 X 2 = P 1,600

SNACKS ---------------------------------------------- 30 X 16 = P480

MEALS --------------------------------------------- 50 X 16 = P800

TOTAL : = P 2,880
