The responsible Supervisor was not available
during this activity but his instruction to use
wood lumber was neglected by his team
resulting in accident
Lack of command on team and failure to
provide resources to execute the work i.e.
sufficient wood lumber was not made available
for the task resulting in worker using pipe piece.
ACCIDENT
Gap between the two ducts was created by
inserting a wood lumber to prevent crush hazard.
But the worker used 8cm long and 3mm thick
hollow pipe section instead of wood lumber
During support installation this piece of pipe
misbalanced and the duct fell on workers hand
