I am 5th year student of University of Southern Mindanao – Kabacan taking up Bachelor of

Science in Civil Engineering.

As one of the requirement in our academic study, I will be conducting a thesis research
entitled “DETERGENT AS AIR-ENTRAINING ADMIXTURE IN CONCRETE”.

In line with this, I would like to seek from your good office the permission to allow me to
borrow equipments namely slump cone molder, tamping rod, scoop, mallet, graduated
cylinder, and sieve number 4, number 100 and 25mm on the ___day of _____, 2020.

I will take full responsibility and will use the equipments with the outmost of care and promise
to replace an equipment in case it will be damaged during the conduct of my research
experiment.
