Jenny Kate M. Guba commonly called "Jenny" a 7 years child was born with Down Syndrome and
weak legs. She is the youngest among the three children.

At present, she speaks on her own words like ate, kuya, mama and papa.

* SELF- HELP SKILLS

Jenny eat independently using fork and spoon. She can comb her hair, dress herself with simple
clothes like blouse, t- shirt and short.

* MOTOR SKILLS

A. Gross Motor

Jenny has a weak legs she cannot jump over an objects with both feet, she can't climb upstairs and
goes downstairs using alternative feet, and she can't walk forward independently.

B. Fine Motor

Sometimes Jenny can hold pencils, crayons properly, cuts small objects, cut curve lines, cut around
picture and sometimes she can build lower 2nd. To 5th. Stage of block building.

* ORIENTATIONAL AND MOBILITY

Jenny can't explore in different area and places, she can't walk independently and she cannot do to
return things on proper place due to her weak legs.

* LANGUAGE AND COMMUNICATION

Jenny can't speak clearly but she can sound it off, she can express herself by facial expression and
gesture to gets.
* SOCIAL AND EMOTIONAL INTERACTION

Jenny is very kind, loving and always wear her sweet smile. Sometimes she know how to follow adult
direction, care for her own belongings and sometimes responds to her name when called.

* ADAPTIVE BEHAVIOR

Jenny she can able to put simple set of clothing. But she independent in others areas of functioning.
And she needs some assistance of an adult on some of her areas of skills.

* COGNITION SKILLS

Jenny can easily understand simple commands related to the task given or assign to her. And she can
identify common parts of her body.

* SUMMATIVE

Jenny is very positive, kind, loving and always wear her sweet smile. She was born with DowN
Syndrome and has weak legs. Despite of her disability she can perform some basic help- self skills
independently. And she can follow simple instruction.

* RECOMMENDATION

A. Continue teaching her to attend present school at (SpEd Center).

B. Language and Speech evaluation is highly recommended.

C. Occupational therapy is also recommended to improve her attention cognitve skills and Language
stimulation.

D. Learning assistance at home.

Jenny Kate M. Guba is a 7 year old child but physically seems like a 3 year old. She was born with
down syndrome and at the same time a blue baby syndrome. The bones in her legs are weak so that
she can't walk independently.

 SELF-HELP SKILLS

Jenny Kate know some basic activities independently such as drinking with the use of glass/cups. She
can identify some shapes and can count 1-10. She can also know how to take off her panties/undies
by her own and some other basic activities.

 MOTOR SKILLS
A. Gross Motor
Jenny Kate can't use her both feet in motor skills activities such as jump over using her both feet. She
can't walk independently, it needs the guidance of others specifically her parents because her both
legs are weak.

B. Fine Motor
Jenny Kate can do some fine motor activities based on what learnings she acquired by her parents.
She can cut an object according to its size and shape but not in a proper way.

 ORIENTATIONAL AND MOBILITY

Jenny Kate can't move herself to one place to another. She can't walk and run independently due to
her weak legs caused by her weak bones that she had.

 LANGUAGE AND COMMUNICATION

Jenny Kate can speak well but sometimes her words are hard to understand. She can only speak the
common words that every child that is younger than her commonly utter. She know a little bit of
English language such as "Yes, No, thank you, please and other basic words.

 SOCIAL AND EMOTIONAL INTERACTION

Jenny Kate is a sensitive child, she cries even in a little things but she is very loving, caring and friendly
child. She loves interacting with peers when it comes in playing indoor and outdoor.
  ADAPTIVE BEHAVIOR

Jenny Kate can easily adapt behavior from others that surrounds her. She can easily learn the things
thought by her parents. Environment easily influence her behavioral status that can affect the way
her traits people in the surrounding.

 COGNITIVE SKILLS

Jenny Kate easily understand command by others specially the command by her parents. She can
easily know to count 1-10 numbers. She has a good capacity in remembering facts that previously
taught to her.

 SUMMATIVE

Jenny Kate Guba was born with down syndrome and a blue baby syndrome. She is a happy kid despite
of his disability. She treat her life as a normal just like the other child. She is very optimistic and an
inspiring child. She is interested in an entertainment stuff just like television and mobile phones
feature. Her disability can't stop her to achieve happiness through the help of her parents and
relatives.

 RECOMMENDATION

1. The parents must send her child to school in order for her to learn more just like a

Normal child.

2. Have a regular checkup for her condition to have enough advices by the expert.

3. Expose her outside to engage social interaction that may help her condition.
