Here by Mr. ___________________________due to the company HSE policy, this is to act as a last
warning letter, informing you that you were observed by HSE representative while violating the
following HSE requirements / rules.

During working at _______________________________________ failure to wear safety helmet, and not

taken that fact into a close consideration, thus leaving us the necessity to inform you that there will be
no other warnings , but your next similar violation will be disciplined by penalty and further similar
actions will result in strict action/termination of your contract by the project management.

I received the above warning letter and will do my best to stop my unsafe actions or omissions of actions
immediately.
