WAIVER

I, __________, of legal age, married, a Filipino and a resident of Villa

Socorro Subd., Purok 5, Libjo Tiwi, Albay, after having been duly sworn to in
accordance with law, hereby depose and say that:

1. I was hired by Smartgeo Surveying and Geomatics(Smartgeo) as

a Safety Officer/ First Aider to be assigned at the gravity survey
and leveling project of Philippine Geothermal Production
Corporation(PGPC) in Tiwi, Albay;

2. As a consequence of my employment, Smartgeo is bound to

pay the necessary contributions of my SSS, Philhealth and PAG-
IBIG.

3. I am waiving said Philealth and PAG-IBIG contributions to be

paid by Smartgeo; and

4. I am further waiving any right to reclaim said Philhealth and

PAG-IBIG contributions against Smartgeo and PGPC in
whatever court of law.

Dear Sir Harley,

I have attached in this letter a notarized quitclaim over my Philhealth and

PAG-IBIG contributions to be paid by Smartgo Surveying and Geomatics. In
said waiver, I have also stated that I am not pursuing any claims arising from
said Philhealth and PAG-IBIG contributions as a result of my assignment by
Smartgeo in PGPC.

May this letter and the attached Notarized Waiver be considered for
whatever purposes it may serve.
