ISSUANCE OF ADMISSION SLIP (OSA FORM 1)

NO ID/NO BAG/ NOT IN PRESCRIBED UNIFORM

1. Whenever a student is not wearing ID, bag, and not in prescribed uniform, the security
guard will ask the student to proceed to the Office of the Student Affairs for the issuance of
admission slip.
2. The Director of the Office of the Student Affairs will interview the student.
3. The student will fill up two (2) copies of admission slip form.
4. The OSA Director will sign the admission slips.
5. The student will present the admission slip – student’s copy to the security guard and to
his/her instructors.

LATE and ABSENT

1. In case of late or absences, the instructor will ask the student to proceed to the Office of
Student Affairs for the issuance of admission slip.
2. The student will write a letter addressed to the instructor and submits the same to the office
of the Student Affairs for notation.
3. In case of sickness/illness, the student must submit a photocopy of Medical Certificate to
the Office of the Student Affairs for notation.
4. In case, there is an excuse letter written and signed by the parent or guardian, the student
will submit it together with the photocopy of parent’s/guardian’s ID to the Office of the
Student Affairs.
5. The Director of the Student Affairs will interview the student.
6. The student will fill up two (2) copies of admission slip forms.
7. The OSA Director will sign the admission slips.
8. The student will present the accomplished admission slip to his/her instructors.
9. The subject instructors will sign it.

REMINDER: DO NOT ACCEPT LATE/ABSENT STUDENT WITHOUT ADMISSION SLIP ISSUED
